As healthcare professionals, we frequently encounter patients presenting with syncope, a transient loss of consciousness (TLOC) due to global cerebral hypoperfusion. Despite its common occurrence, the diagnosis and management of syncope remain challenging due to its multifactorial etiologies.
Syncope can be classified into three main categories: reflex (neurally mediated), orthostatic hypotension, and cardiac syncope. Reflex syncope is the most common type and includes vasovagal, situational, and carotid sinus syncope. Orthostatic hypotension is typically caused by medication, volume depletion, or autonomic dysfunction. Cardiac syncope can be due to arrhythmias or structural heart disease, and it carries the highest risk of mortality.
The initial evaluation of syncope should include a detailed history and physical examination. Specific questions about the circumstances of the event, prodromal symptoms, and family history of sudden death can provide crucial clues. Orthostatic blood pressure measurements and an ECG should be performed in all patients. Further testing, such as echocardiography, tilt-table testing, or event monitoring, should be guided by the initial evaluation.
Management of syncope is primarily aimed at preventing recurrence and depends on the underlying cause. In reflex syncope, lifestyle modifications and physical counterpressure maneuvers are often effective. For orthostatic hypotension, adjusting medications, increasing fluid and salt intake, and use of compression stockings may be beneficial. Cardiac syncope may require medication, catheter ablation, or implantation of a pacemaker or defibrillator.
Syncope is a complex clinical syndrome with a broad differential diagnosis. A systematic approach to evaluation and management can help healthcare professionals provide optimal care for patients with syncope. Continued research is needed to further elucidate the pathophysiology of syncope and to develop more effective treatment strategies.
