Acute chest pain (ACP) is a common symptom that presents in clinical practice, often posing a diagnostic challenge. This symptom may indicate serious cardiovascular conditions, but it can also be a manifestation of gastrointestinal, musculoskeletal, or psychological disorders. Therefore, it is crucial to have a comprehensive understanding and approach to diagnosing ACP.
The initial evaluation of ACP should focus on identifying life-threatening conditions. A thorough history taking, physical examination, and basic investigations such as electrocardiogram (ECG) and chest radiographs are essential. The patient's age, risk factors, and the nature of the pain can guide the clinician towards certain diagnoses.
Further diagnostic testing is guided by the initial assessment. In cases where acute coronary syndrome (ACS) is suspected, cardiac biomarkers and stress testing may be required. For suspected pulmonary conditions, computed tomography (CT) and ventilation-perfusion scans can be beneficial. Gastrointestinal causes may warrant endoscopy or abdominal imaging.
Risk stratification tools can aid in the management of patients with ACP. Tools such as the HEART score and the TIMI risk score can help identify patients at high risk for adverse events, aiding in the decision for hospital admission or discharge.
Shared decision making is essential in managing ACP. After diagnostic testing and risk stratification, clinicians should discuss the potential benefits, risks, and uncertainties of different management strategies with patients. This approach ensures that patient values and preferences are incorporated into the management plan.
Diagnosing ACP requires a comprehensive approach, including initial assessment, diagnostic testing, risk stratification, and shared decision making. By adopting this method, clinicians can ensure that patients receive the most appropriate and effective care, ultimately improving patient outcomes.
