The evaluation of fever should be systematic and patient-centered. The history should focus on the onset, duration, pattern, and associated symptoms. The physical examination should be thorough, emphasizing signs that may suggest specific etiologies. Laboratory investigations, including complete blood count, C-reactive protein, and cultures, may provide additional information. Imaging studies can also be useful in certain scenarios.
Management of fever depends on the underlying cause. In infectious causes, antimicrobial therapy is the mainstay of treatment. Inflammatory or autoimmune disorders may require immunosuppressive agents. Symptomatic management of fever with antipyretics should be considered, especially in patients with discomfort or significant comorbidities.
In certain populations, such as the elderly or immunocompromised, fever may be absent or subtle despite serious underlying conditions. Hence, a high index of suspicion is necessary. Similarly, fever of unknown origin (FUO) presents a diagnostic challenge, necessitating a broad differential and extensive workup.

A comprehensive, systematic approach to evaluation can help clinicians identify the underlying cause and initiate appropriate management. By understanding the nuances of fever assessment, clinicians can enhance patient care and outcomes.
