Peptic ulcer disease (PUD) represents a significant cause of morbidity and mortality worldwide. The traditional approach to managing PUD has been transformed in recent years, thanks to advancements in diagnosis, treatment, and prevention strategies. This article will provide a comprehensive review of these advancements.
Endoscopy remains the gold standard for diagnosing PUD. However, the advent of less invasive diagnostic tools like serological tests for Helicobacter pylori and urea breath tests has made early detection easier. Additionally, capsule endoscopy and endoscopic ultrasound have emerged as valuable tools for assessing the severity and complications of PUD.
Traditionally, treatment of PUD involved acid suppression, antacids, and lifestyle modifications. Today, the eradication of H. pylori with triple or quadruple therapy has become the cornerstone of PUD management. Furthermore, the development of proton pump inhibitors (PPIs) has revolutionized the treatment, offering superior acid suppression and healing rates. Recently, the advent of vonoprazan, a potassium-competitive acid blocker, has shown promising results in terms of efficacy and safety.
Prevention of PUD involves addressing modifiable risk factors such as non-steroidal anti-inflammatory drug (NSAID) use and smoking. Prophylactic use of PPIs or H2 receptor blockers is recommended for high-risk patients on NSAIDs. Moreover, patient education about the disease, its causes, and the importance of adhering to treatment is now considered an integral part of PUD management.
The management of PUD has seen significant advancements, with improvements in diagnostic techniques, treatment modalities, and preventive strategies. These advancements have led to better patient outcomes and a reduction in disease complications. As healthcare professionals, staying abreast of these developments is essential to provide optimal care to patients with PUD.
