Introduction

Enteric fever, predominantly caused by Salmonella Typhi and Paratyphi, remains a significant health concern worldwide. Its management in clinical practice requires a multi-faceted approach, incorporating diagnostic strategies, appropriate antibiotic therapy, and preventive measures.

Diagnostic Strategies

Early diagnosis of enteric fever is crucial for effective management. Blood culture remains the gold standard for diagnosis, though it has limitations, including the need for laboratory facilities and time for culture growth. Rapid diagnostic tests, such as Typhidot and Tubex, offer promising alternatives, providing results within hours.

Antibiotic Therapy

Increasing antimicrobial resistance necessitates careful selection of antibiotics. Empirical therapy with third-generation cephalosporins or fluoroquinolones is often initiated, pending culture results. However, due to widespread resistance, it is critical to adjust treatment based on susceptibility patterns. In resistant cases, azithromycin has been found effective.

Preventive Measures

Prevention is an integral part of enteric fever management. This includes safe food and water practices, personal hygiene, and vaccination. Typhoid vaccines, both conjugate and polysaccharide, are recommended in endemic regions and for travelers. The newer conjugate vaccine has shown superior efficacy and duration of protection.

Stewardship and Surveillance

Antimicrobial stewardship programs can play a significant role in optimizing antibiotic use and minimizing resistance. Moreover, robust surveillance systems are essential to monitor disease trends, detect outbreaks, and guide vaccination strategies.

Conclusion

Effective management of enteric fever encompasses timely diagnosis, appropriate antibiotic therapy, and preventive measures. The challenge of antimicrobial resistance underscores the importance of stewardship and surveillance. As clinicians, our aim should be not only to treat the disease but also to prevent it, thereby reducing the global burden of enteric fever.
