Chronic cough, defined as a cough persisting for more than eight weeks, poses a significant diagnostic challenge in clinical practice. This article aims to provide a comprehensive approach to its diagnosis and management.
Chronic cough can be attributed to a variety of conditions, including asthma, gastroesophageal reflux disease (GERD), and postnasal drip syndrome. A thorough history, physical examination, and relevant investigations are crucial to identify the underlying cause.
The diagnostic approach involves a stepwise evaluation. Empiric treatment trials for the most common causes can be considered. If the cough persists, more specific testing may be warranted, such as bronchoscopy or high-resolution computed tomography (HRCT).
Treatment should be tailored to the underlying cause. For instance, proton pump inhibitors (PPIs) are recommended for GERD-associated cough, while inhaled corticosteroids are the treatment of choice for asthma-related cough. In refractory cases, non-pharmacological interventions, including speech therapy and behavioral therapy, may be beneficial.
Emerging therapies, such as neurokinin-1 receptor antagonists and P2X3 antagonists, show promise in treating refractory chronic cough. However, further research is needed to establish their efficacy and safety profile.
Chronic cough is a complex clinical entity requiring a systematic approach for effective management. By understanding the etiology, adopting a stepwise diagnostic plan, and utilizing targeted treatment strategies, clinicians can significantly improve patient outcomes. The potential of emerging therapies also offers hope for patients with refractory chronic cough. Continued research in this area is essential to further enhance our understanding and management of this challenging condition.
