Hypertensive emergencies, characterized by severe elevations in blood pressure (BP) surpassing 180/120 mmHg, accompanied by evidence of impending or progressive target organ dysfunction, represent a critical condition that necessitates immediate therapeutic intervention. This article aims to provide a comprehensive overview of the diagnosis, management, and treatment of hypertensive emergencies.
Early identification of hypertensive emergencies is crucial to prevent irreversible organ damage. Symptoms may include severe headache, visual disturbances, chest pain, or dyspnea. Diagnostic methods include a thorough physical examination, laboratory tests, and imaging studies to assess the extent of organ damage. The presence of end-organ damage differentiates hypertensive emergencies from urgencies, which also present with high BP but without associated acute target organ damage.
Immediate reduction of BP is paramount in hypertensive emergencies. However, care should be taken to avoid overly rapid reduction, which can lead to hypoperfusion and worsen organ damage. The initial goal is to reduce mean arterial pressure by no more than 25% within the first hour, followed by a gradual reduction over the next 24-48 hours. This can be achieved using intravenous antihypertensive agents under close monitoring.
Several intravenous antihypertensive medications are available for the treatment of hypertensive emergencies, including nitroprusside, labetalol, and nicardipine. The choice of medication depends on the specific clinical scenario, patient characteristics, and potential side effects. Once the BP is controlled, transition to oral antihypertensive therapy should be initiated.

A thorough understanding of the diagnostic criteria, management strategies, and treatment options is essential for healthcare professionals to effectively manage these complex patients.
