Parkinson's Disease (PD) is a chronic, progressive neurodegenerative disorder characterized by motor and non-motor symptoms. A comprehensive understanding of its pathophysiology and clinical management is vital for healthcare professionals.

Pathophysiology of Parkinson's Disease

PD is primarily associated with the loss of dopaminergic neurons in the substantia nigra pars compacta and the presence of Lewy bodies. This neuronal loss leads to an imbalance in the direct and indirect pathways of the basal ganglia, resulting in the characteristic motor symptoms of PD. The pathogenesis of PD is multifactorial, involving genetic susceptibility, environmental factors, and aging.

Clinical Manifestations

The clinical presentation of PD includes both motor and non-motor symptoms. The cardinal motor symptoms are bradykinesia, rigidity, rest tremor, and postural instability. Non-motor symptoms may precede motor symptoms and include olfactory dysfunction, sleep disorders, mood disorders, and autonomic dysfunction.

Diagnostic Approach

Diagnosis of PD is mainly clinical, based on the presence of characteristic motor symptoms and response to dopaminergic therapy. Neuroimaging, such as DaTSCAN, can support the diagnosis in uncertain cases. It is also important to differentiate PD from other parkinsonian syndromes.

Management of Parkinson's Disease

Management of PD is symptomatic and individualized, aiming to improve quality of life. Pharmacotherapy, including levodopa and dopamine agonists, is the mainstay of treatment. Non-pharmacological interventions, such as physiotherapy and occupational therapy, play a crucial role. Deep brain stimulation is an option for patients with advanced disease and motor complications.

Understanding the pathophysiology of PD is fundamental to its clinical management. Early recognition of symptoms, accurate diagnosis, and individualized management can significantly improve the quality of life of patients with PD. Continued research is required to further elucidate the pathogenesis and to develop disease-modifying therapies.
