Pulmonary hypertension (PH) is a pathophysiological disorder characterized by elevated pulmonary arterial pressure. It's a progressive, life-threatening condition that requires a nuanced understanding for effective management.
PH is primarily a disease of the small pulmonary arteries. The endothelial dysfunction leads to vasoconstriction, vascular remodeling, and thrombosis. These changes result in increased pulmonary vascular resistance and subsequently elevated pulmonary arterial pressure.
Patients with PH often present with nonspecific symptoms such as dyspnea, fatigue, and syncope. As the disease progresses, signs of right heart failure may become evident. The nonspecific nature of symptoms often leads to delayed diagnosis and treatment.
The diagnostic approach involves a combination of clinical evaluation, echocardiography, and right heart catheterization. Echocardiography is used as a screening tool, while right heart catheterization confirms the diagnosis and helps assess disease severity.
Treatment of PH involves addressing the underlying cause, managing symptoms, and slowing disease progression. Therapies include vasodilators, endothelin receptor antagonists, phosphodiesterase-5 inhibitors, and prostacyclin analogs. In severe cases, lung transplantation may be considered.
Understanding the complexities of PH is crucial for timely diagnosis and effective management. As healthcare professionals, we must stay abreast with the latest developments in its pathophysiology, diagnostic approaches, and treatment modalities to provide the best care for our patients.
