Thyroid nodules represent a common clinical problem, with an increasing prevalence due to enhanced imaging techniques. Their evaluation and management can be challenging for healthcare professionals due to the risk of malignancy associated with them.
Thyroid nodules are more common in females, the elderly, and individuals exposed to radiation. Most nodules are asymptomatic and discovered incidentally. However, some may present with compressive symptoms or thyroid dysfunction.
The initial evaluation of a thyroid nodule involves a comprehensive history and physical examination, followed by thyroid function tests. High-resolution ultrasound is the imaging modality of choice, providing information on nodule size, number, echogenicity, and vascularity. Suspicious ultrasound features may warrant a fine-needle aspiration biopsy (FNAB).
FNAB is the most reliable method for differentiating benign from malignant nodules. The Bethesda System for Reporting Thyroid Cytopathology is widely used to interpret FNAB results, guiding further management. Molecular testing can be employed in indeterminate cases.
Management depends on the size, FNAB results, and patient preference. Options include surveillance, thyroidectomy, and nonsurgical treatments like radiofrequency ablation or ethanol injection. High-risk patients may require a total thyroidectomy or radioactive iodine therapy.
Assessing thyroid nodules requires a systematic approach, encompassing clinical assessment, appropriate use of diagnostic modalities, and individualized management strategies. Ongoing research and advancements in molecular diagnostics hold promise for improving the accuracy of thyroid nodule evaluation, contributing to more personalized patient care.
