History taking is the cornerstone in the evaluation of abdominal pain. Key elements include pain characteristics, associated symptoms, risk factors, and past medical history. The temporal pattern of pain (acute, chronic, or recurrent) can also provide valuable clues. For instance, sudden onset of severe pain might suggest perforation or infarction, while chronic pain could indicate functional disorders or malignancy.
A thorough physical examination can further narrow down the differential diagnosis. Important aspects include general appearance, abdominal inspection, palpation, percussion, and auscultation. Special attention should be given to signs of peritonitis, which necessitate immediate surgical consultation.
Diagnostic tests should be used judiciously based on the pre-test probability of specific conditions. Basic tests include complete blood count, liver function tests, lipase, and urinalysis. Imaging studies like ultrasound or computed tomography can be considered in certain cases. Invasive procedures such as endoscopy or laparoscopy should be reserved for when the diagnosis remains unclear after initial evaluation.
Each abdominal quadrant has its own set of differential diagnoses. For example, right upper quadrant pain could indicate gallbladder or liver disease, while left lower quadrant pain might suggest diverticulitis or ovarian pathology. It is essential to maintain a broad differential and consider atypical presentations of common diseases.
In conclusion, a systematic approach to abdominal pain can facilitate accurate diagnosis and timely management. By integrating a detailed history, careful physical examination, and appropriate diagnostic tests, clinicians can effectively decipher the enigma of abdominal pain.
