Various conditions can cause elevated liver enzymes, including alcoholic and non-alcoholic fatty liver disease, viral hepatitis, autoimmune liver disease, drug-induced liver injury, and less commonly, genetic disorders like hemochromatosis and Wilson's disease. A thorough history, physical examination, and appropriate investigations are crucial in narrowing down the differential diagnosis.
The management of elevated liver enzymes depends on the underlying cause. Lifestyle modifications are often recommended for patients with fatty liver disease, while antiviral therapy may be needed for viral hepatitis. For drug-induced liver injury, identifying and discontinuing the offending agent is paramount. In some cases, a liver biopsy may be necessary to confirm the diagnosis and guide treatment.
Regular monitoring of liver enzymes is crucial for assessing the response to treatment and detecting potential complications. The frequency of follow-up and additional investigations will depend on the severity of liver disease and the patient's overall clinical status.
Elevated liver enzymes pose a diagnostic challenge in clinical practice. A systematic approach that includes a thorough evaluation, appropriate differential diagnosis, targeted management, and regular monitoring can help clinicians effectively manage these patients. Continued research and advancements in this field will undoubtedly enhance our understanding and management of liver diseases.
