The first step involves a thorough history and physical examination. Key elements to consider include pain characteristics, associated symptoms, risk factors, and physical findings. This information aids in stratifying patients into low, intermediate, and high-risk categories.
Acute coronary syndrome (ACS) is a primary concern, but other differential diagnoses like pulmonary embolism, aortic dissection, and pericarditis should be considered. Gastrointestinal disorders such as gastroesophageal reflux disease (GERD) and musculoskeletal causes may also present as chest pain.
Electrocardiogram (ECG) and troponin levels remain the cornerstone of ACS diagnosis. However, further tests may be needed, such as chest radiography, computed tomography (CT), or echocardiography, depending on the suspected differential diagnosis.
Management depends on the underlying cause. ACS requires urgent revascularization and antiplatelet therapy. Pulmonary embolism and aortic dissection necessitate immediate anticoagulation or surgical intervention, respectively. Non-cardiac causes should be managed accordingly.
