Chronic cough, defined as a cough persisting beyond eight weeks, poses a significant diagnostic and therapeutic challenge in clinical practice. It can be a symptom of an underlying condition or a disease entity in itself. This article aims to provide a comprehensive approach to its management.

Identifying the Underlying Cause

Identifying the cause is the first step in managing chronic cough. Common etiologies include gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS) secondary to rhinosinus conditions. Less common causes, such as lung cancer, should also be considered in high-risk patients. A detailed history, physical examination, and appropriate diagnostic tests are crucial in this phase.

Treatment Strategies

Treatment should be directed at the underlying cause. For example, proton pump inhibitors may be beneficial in GERD-associated cough, while inhaled corticosteroids are the treatment of choice for asthma-related cough. In cases where chronic cough is a disease in itself, centrally acting agents like gabapentin and amitriptyline have shown efficacy.

Challenges and Future Directions

Despite a comprehensive approach, a subset of patients remains with idiopathic chronic cough. In these cases, a multidisciplinary approach involving pulmonologists, gastroenterologists, and otolaryngologists may be beneficial. Future research should focus on understanding the pathophysiology of chronic cough better and developing targeted therapies.
