This review synthesizes current evidence on the epidemiology, pathophysiology, risk factors, clinical features, and management of obesity through a public health lens, with an emphasis on reducing inequities. It explores the mechanisms by which social determinants and environmental exposures contribute to obesity risk, evaluates emerging therapies and policy interventions, and provides guideline-based recommendations for clinicians and policymakers. The article underscores the necessity of integrated, equity-focused public health strategies to mitigate obesity’s burden among vulnerable groups.
Obesity, defined as excessive fat accumulation adversely affecting health, has reached pandemic proportions globally. Beyond its well-established association with metabolic, cardiovascular, and oncologic diseases, obesity is increasingly recognized as both a cause and consequence of social and environmental inequities. These inequities manifest through differential access to resources, healthcare, healthy foods, and safe environments, disproportionately impacting racial, ethnic, and socioeconomic minorities. A nuanced understanding of the interplay between obesity and inequity is essential for clinicians and public health professionals seeking to implement effective, sustainable interventions. This article aims to provide a comprehensive, evidence-based overview of public health approaches that address both obesity and its associated social and environmental determinants.
Obesity prevalence has escalated dramatically in recent decades, with the World Health Organization estimating that over 650 million adults are obese worldwide. In the United States, the CDC reports that 42.4% of adults and 19.3% of children and adolescents are affected. However, these figures mask pronounced disparities: non-Hispanic Black and Hispanic populations, as well as individuals residing in low-income and rural areas, bear the highest burden. Obesity contributes to increased risk of type 2 diabetes, hypertension, cardiovascular disease, certain cancers, and reduced life expectancy, amplifying healthcare costs and straining public health systems. The disease burden extends beyond physical morbidity, encompassing psychosocial consequences such as stigma, discrimination, and reduced socioeconomic mobility, further entrenching existing inequities.
The pathophysiology of obesity involves a dysregulation of energy balance, governed by complex neuroendocrine, genetic, and environmental factors. Adipose tissue acts as an endocrine organ, secreting adipokines and pro-inflammatory cytokines that promote insulin resistance, chronic inflammation, and metabolic dysfunction. Social and environmental determinants—such as chronic stress, food insecurity, urban design, and exposure to obesogenic marketing—modulate physiological pathways, exacerbating susceptibility in high-risk populations. Epigenetic modifications induced by early-life adversity or poor nutrition may perpetuate risk across generations, underscoring the need for upstream interventions.
Obesity risk is shaped by a constellation of modifiable and non-modifiable factors. Genetics account for an estimated 40–70% of individual susceptibility, but environmental and social determinants are increasingly recognized as key drivers. These include: limited access to healthy foods (food deserts), unsafe neighborhoods restricting physical activity, targeted advertising of unhealthy products in low-income communities, lower educational attainment, psychosocial stress, and limited access to healthcare. Clinical manifestations are often compounded by social determinants: individuals from marginalized backgrounds may present later, with more advanced disease and greater functional impairment, reflecting barriers to timely care and preventive services.
Diagnosis of obesity relies primarily on BMI, supplemented by waist circumference and assessment of body fat distribution. However, these metrics may inadequately capture risk in certain populations. Comprehensive evaluation should include screening for comorbidities, assessment of psychosocial factors, and identification of social and environmental barriers to healthy behaviors. Culturally sensitive, equity-oriented tools are essential for accurate diagnosis and risk stratification, particularly in diverse populations.
Effective management of obesity requires a multi-tiered approach, integrating lifestyle modification, pharmacotherapy, and—when indicated—bariatric surgery. Nutrition counseling, physical activity promotion, and behavioral interventions form the cornerstone of therapy. However, traditional individual-based interventions may inadequately address underlying social and environmental determinants. Community-based programs, policy reforms (e.g., taxing sugar-sweetened beverages, improving urban infrastructure, incentivizing healthy food retail), and integration with social services can enhance effectiveness and sustainability. Clinicians should employ trauma-informed, patient-centered care, recognizing the impact of stigma and systemic barriers on engagement and outcomes.
Recent advances in obesity management include the development of novel pharmacotherapies (e.g., GLP-1 receptor agonists such as semaglutide), minimally invasive endoscopic procedures, and personalized medicine approaches leveraging genetic and metabolic profiling. Digital health platforms and telemedicine expand access to care, particularly in underserved areas. Public health innovations—such as healthy food prescription programs, school-based interventions, and policies promoting active transportation—demonstrate promise in reducing inequities. Ongoing research is elucidating the role of microbiome modulation, epigenetics, and social network interventions in obesity prevention and treatment.
Major guidelines, including those from the American Heart Association, the Obesity Society, and the World Health Organization, emphasize a comprehensive, multidisciplinary approach to obesity prevention and management. Recommendations include routine screening for obesity and related comorbidities, culturally tailored interventions, addressing social determinants of health, and advocating for policy changes that promote health equity. Clinicians are encouraged to collaborate with community organizations and public health agencies to implement population-level strategies, reduce stigma, and ensure equitable access to evidence-based care.
Obesity-associated social and environmental inequities represent a critical public health challenge requiring coordinated, multi-level interventions. Addressing the root causes—structural inequities, environmental exposures, and social determinants—is paramount to reducing disparities in obesity prevalence and outcomes. Evidence-based public health strategies, informed by recent advances and guideline recommendations, can foster healthier environments, empower vulnerable populations, and promote equity in obesity prevention and care. Ongoing research, policy innovation, and cross-sector collaboration are essential for sustained progress.
