Introduction

Enteric fever, predominantly caused by Salmonella Typhi and Salmonella Paratyphi, is a systemic illness that continues to pose significant health challenges globally. This article presents a comprehensive and updated approach to the diagnosis and treatment of this disease.

Diagnosis of Enteric Fever

Traditionally, the diagnosis of enteric fever has relied heavily on blood culture which, despite being the gold standard, has a sensitivity of only 40-60%. Recent advancements in molecular diagnostics, such as Polymerase Chain Reaction (PCR) and Next-Generation Sequencing (NGS), have shown promise in improving diagnostic accuracy. These techniques can detect bacterial DNA directly from patient samples, thereby increasing sensitivity and specificity. Serology tests, such as the Widal test, though less reliable, may be used in resource-limited settings.

Treatment of Enteric Fever

Antibiotic therapy remains the mainstay of treatment for enteric fever. However, the emergence of multi-drug resistant strains necessitates careful antibiotic selection. Current guidelines recommend third-generation cephalosporins, such as ceftriaxone or cefixime, or fluoroquinolones, like ciprofloxacin, for initial empirical therapy. In cases of confirmed multi-drug resistance, azithromycin has shown efficacy. It is crucial to adjust therapy based on culture and sensitivity results to prevent treatment failure and further resistance.

Prevention and Control

Prevention strategies include improved sanitation, safe food and water practices, and vaccination. Two vaccines, Ty21a (oral) and Vi polysaccharide (injectable), are available and recommended for high-risk populations and travelers to endemic areas. However, vaccination should not replace basic hygiene practices as it provides only moderate protection.

Enteric fever remains a significant health burden, particularly in low-resource settings. While diagnostic and treatment strategies continue to evolve, emphasis should be placed on prevention and control measures to reduce the incidence of this disease.
