Low vision represents a significant public health challenge, impairing daily function and quality of life for millions worldwide. This review synthesizes current knowledge on the epidemiology, pathophysiology, risk factors, clinical features, diagnostic approaches, and evidence-based strategies for functional adaptation in patients with low vision. Emphasis is placed on recent advances, emerging therapies, and guideline-driven management to inform best practices for clinicians dedicated to optimizing patient independence and well-being.
Low vision is defined as a visual impairment not fully correctable by standard glasses, contact lenses, medication, or surgery, resulting in a significant impact on daily activities. This condition is distinct from blindness and encompasses a spectrum of visual deficits that impede reading, mobility, and self-care. As the global population ages and the prevalence of chronic ocular diseases rises, the clinical importance of low-vision rehabilitation and functional adaptation strategies has become increasingly prominent. The following review explores contemporary scientific understanding and clinical management approaches for low-vision patients, integrating recent research and guideline recommendations to support evidence-based care.
Globally, an estimated 253 million people live with visual impairment, of whom approximately 217 million have low vision and 36 million are blind. The prevalence of low vision escalates with age, making it a critical concern in geriatric populations. Age-related macular degeneration (AMD), diabetic retinopathy, glaucoma, and uncorrected refractive errors constitute the leading causes. Socioeconomic disparities, limited access to eye care, and increasing life expectancy contribute to the expanding burden. In high-income countries, the incidence of low vision has stabilized due to improved screening and management, whereas low- and middle-income nations continue to experience a rising trend, highlighting the need for scalable adaptation strategies.
The pathophysiological mechanisms underlying low vision are heterogeneous, reflecting the diversity of ocular diseases involved. Central vision loss, as in AMD, impairs tasks requiring fine detail, such as reading and facial recognition. Peripheral vision loss, often seen in glaucoma or retinitis pigmentosa, compromises mobility and spatial orientation. Diabetic retinopathy and retinal vein occlusion may cause patchy or fluctuating vision. Functional impairment stems from deficits in contrast sensitivity, color discrimination, depth perception, and visual field. Neuroplasticity and compensatory mechanisms such as the use of preferred retinal loci play a role in adaptation, but often require targeted rehabilitation for optimal functional outcomes.
Risk factors for low vision development include advanced age, family history of ocular diseases, diabetes mellitus, hypertension, smoking, and prolonged exposure to ultraviolet light. Systemic conditions such as cardiovascular disease, metabolic syndrome, and neurodegenerative disorders have been associated with higher risk. Certain medications and ocular trauma may also contribute. Social determinants such as limited access to eye care, lower educational attainment, and poverty exacerbate vulnerability, underscoring the importance of preventive strategies and early intervention in high-risk populations.
Patients with low vision present with a variety of symptoms, including blurred vision, difficulty reading print, inability to recognize faces, impaired mobility, and increased risk of falls. Visual acuity may range from mild to profound impairment, with or without field loss. Clinical assessment should encompass best-corrected visual acuity, contrast sensitivity, color vision, visual field testing, and functional evaluation of daily tasks. Subjective complaints often underestimate the true extent of disability, making comprehensive assessment crucial for tailored intervention.
Diagnosis of low vision requires a multidisciplinary approach, integrating ophthalmologic examination with functional vision assessment. Standardized tools such as the Early Treatment Diabetic Retinopathy Study (ETDRS) chart, Pelli-Robson contrast sensitivity chart, and Goldmann visual field testing are widely used. Functional questionnaires including the National Eye Institute Visual Function Questionnaire (NEI VFQ-25) provide insight into patient-perceived disability. Neuro-ophthalmologic evaluation may be warranted to exclude non-ocular causes. Collaboration with low-vision rehabilitation specialists enables comprehensive planning and ongoing assessment of adaptation strategies.
Functional adaptation for low-vision patients centers on maximizing residual vision and promoting independence. Key strategies include the prescription of low-vision aids (LVAs) such as magnifiers, telescopes, electronic reading devices, and high-contrast materials. Environmental modifications improved lighting, high-contrast markings, and tactile cues enhance safety and mobility. Vision rehabilitation programs, involving occupational therapy and orientation-mobility training, are critical for developing compensatory skills. Psychological support and counseling address the emotional impact of visual impairment, reducing depression and social isolation. Multidisciplinary care teams, including ophthalmologists, optometrists, rehabilitation therapists, and social workers, are essential for comprehensive management.
Recent years have witnessed significant progress in low-vision adaptation, driven by technological innovation and a deeper understanding of neuroplasticity. Electronic visual aids including portable video magnifiers, wearable smart glasses, and smartphone applications offer customizable solutions for reading and navigation. Artificial intelligence-based software can convert text to speech or provide real-time object recognition. Research into retinal prostheses ("bionic eyes") and optogenetic therapies holds promise for restoring limited vision in select patients. Virtual reality-based rehabilitation platforms are being explored to enhance neuroadaptive training, with preliminary evidence suggesting improvements in functional outcomes. However, accessibility and cost remain barriers to widespread adoption.
Leading ophthalmologic societies advocate early referral to low-vision rehabilitation for all patients with irreversible visual impairment. Guidelines emphasize individualized assessment, interdisciplinary collaboration, and the integration of both optical and non-optical aids. Regular follow-up is recommended to assess adaptation, monitor for emotional distress, and update interventions as needs evolve. Patient education and empowerment are central tenets, with a focus on maximizing quality of life, safety, and autonomy. In resource-limited settings, community-based rehabilitation and peer support programs are encouraged to bridge gaps in care.
Low vision imposes a profound clinical and psychosocial burden, necessitating a comprehensive, patient-centered approach to functional adaptation. Advances in technology, neurorehabilitation, and collaborative care have expanded the therapeutic landscape, offering renewed hope for improved quality of life. Ongoing research and policy efforts are needed to enhance accessibility, refine interventions, and ensure equitable care for all patients living with low vision. For clinicians, early recognition, tailored rehabilitation, and compassionate support remain the cornerstones of effective management.
