Acute chest pain (ACP) is a common symptom that often presents diagnostic challenges in clinical practice. The differential diagnosis is broad, ranging from benign conditions to life-threatening emergencies. This article aims to provide a comprehensive approach to the differential diagnosis of ACP.
The clinician's first task is to stabilize the patient and rule out life-threatening conditions. A focused history, physical examination, and initial diagnostic tests, such as electrocardiogram (ECG) and cardiac biomarkers, are critical in this phase.
Distinguishing between cardiac and non-cardiac chest pain is crucial. Cardiac chest pain, often due to ischemic heart disease, usually presents with characteristic features. However, atypical presentations are not uncommon, particularly in women, the elderly and patients with diabetes. Non-cardiac chest pain can be due to gastrointestinal, pulmonary, musculoskeletal or psychological conditions.
Further diagnostic testing should be guided by the pre-test probability of disease. For patients with an intermediate probability of coronary artery disease (CAD), stress testing can be useful. For those with a high probability of CAD or unstable symptoms, coronary angiography is usually indicated. For non-cardiac chest pain, appropriate diagnostic tests should be chosen based on the suspected underlying condition.
Management should be tailored to the underlying cause of ACP. Patients with confirmed or suspected CAD should be managed according to current guidelines, which may include medications, lifestyle modifications, and revascularization procedures. Non-cardiac chest pain should be managed based on the specific diagnosis.
ACP represents a diagnostic challenge due to its broad differential diagnosis. A systematic approach, incorporating initial assessment, differentiation between cardiac and non-cardiac chest pain, guided diagnostic testing and tailored management, can assist clinicians in achieving an accurate diagnosis and optimal patient outcomes.
