The transition from hospital to home remains a vulnerable period for patients, especially those with complex medical needs. Nurse-led transitional care models have emerged as effective interventions to reduce readmissions and improve outcomes following early hospital discharge. This article reviews the current evidence, mechanisms, and clinical implications of nurse-led transitional care through a case-based learning approach, synthesizing recent advances and guideline-driven practices to inform clinicians and healthcare teams.
Early hospital discharge is increasingly common due to healthcare cost constraints and bed shortages, yet it carries significant risks for patient safety and continuity of care. Transitional care bridges the gap between inpatient and outpatient settings, aiming to reduce adverse events and rehospitalizations. Nurse-led transitional care leverages the unique skills of nursing professionals in patient education, care coordination, and early intervention, making it a cornerstone of modern discharge planning. This review focuses on the principles, implementation, and outcomes associated with nurse-led transitional care using a case-based approach to highlight key clinical concepts.
Hospital readmission rates within 30 days of discharge remain a key quality metric, with rates ranging from 13% to 20% in many healthcare systems. Chronic diseases—such as heart failure, chronic obstructive pulmonary disease (COPD), and diabetes—are disproportionately represented in readmitted populations. The economic burden is substantial; in the United States alone, unplanned readmissions cost billions annually. Vulnerable groups, including the elderly and those with multiple comorbidities, are particularly at risk, underscoring the need for improved transitional care strategies.
The pathophysiologic basis for adverse events post-discharge centers on the abrupt discontinuity in care, medication errors, and insufficient patient education. Physiological instability at discharge, unresolved symptoms, and inadequate follow-up contribute to clinical deterioration. Nurse-led transitional care addresses these factors by providing ongoing assessment, supporting medication adherence, and facilitating early recognition of warning signs, thus mitigating the risk of decompensation.
Key risk factors for poor outcomes after early discharge include advanced age, polypharmacy, low health literacy, limited social support, and high disease complexity. Social determinants of health, such as socioeconomic status and access to primary care, further modulate risk and must be systematically assessed during discharge planning.
Patients at risk for post-discharge complications may present with new or worsening symptoms, medication confusion, or inability to perform activities of daily living. Early signs of clinical instability include dyspnea, altered mental status, and poor oral intake. Nurse-led transitional care models emphasize structured symptom monitoring and frequent patient contact to detect and respond to these clinical features promptly.
Identifying patients who will benefit most from transitional care involves comprehensive risk stratification at discharge. Tools such as the LACE index and the HOSPITAL score aid in predicting readmission risk. Nurses conduct detailed assessments, including evaluation of cognitive function, functional status, and psychosocial factors, to individualize transitional care plans and anticipate barriers to recovery.
Effective nurse-led transitional care includes a continuum of interventions: discharge education, medication reconciliation, care plan development, and post-discharge follow-up (telephonic or in-person). Case management often involves coordination with primary care, specialists, and community resources. Protocol-driven symptom checklists and structured communication (e.g., teach-back methods) enhance patient understanding and promote self-management. Home visits and remote monitoring are integral in high-risk populations.
Recent innovations in transitional care leverage digital health tools, such as telehealth platforms and mobile applications, to extend nurse-led interventions beyond traditional home visits. Artificial intelligence-assisted risk prediction and remote patient monitoring enable proactive management of clinical deterioration. Evidence from randomized controlled trials supports the efficacy of structured nurse-led programs in reducing readmissions, particularly when combined with technology-enabled care pathways.
Professional societies, including the American Heart Association and the National Institute for Health and Care Excellence (NICE), recommend structured transitional care interventions for high-risk patients. Guidelines emphasize early risk assessment, individualized discharge planning, and post-discharge follow-up within 48–72 hours. Multidisciplinary collaboration, led by nurses, is a critical component of recommended care models. Adherence to these guidelines is associated with improved patient satisfaction, reduced readmissions, and better functional outcomes.
Nurse-led transitional care after early hospital discharge is a clinically validated strategy to optimize patient outcomes, minimize preventable readmissions, and improve continuity of care. Through comprehensive risk assessment, patient-centered education, and coordinated post-discharge follow-up, nurses play a pivotal role in bridging hospital and community care. Emerging digital health tools and evidence-based protocols continue to enhance the efficacy of these interventions. Adoption of guideline-driven, nurse-led transitional care models should be prioritized to address gaps in post-discharge support and advance health system performance.
