Organ-sparing surgical techniques in the context of maternal health represent a significant advancement in gynecological and obstetric surgery. This review critically examines recent progress in maternal organ-sparing approaches, focusing on their epidemiology, pathophysiologic rationale, clinical indications, diagnostic algorithms, therapeutic strategies, and evolving guidelines. Emphasis is placed on the balance between maternal safety, fetal outcomes, and organ preservation, referencing contemporary evidence and expert consensus to inform clinical practice.
The evolution of surgical practice in maternal care has increasingly prioritized organ preservation to optimize short- and long-term outcomes for both mother and fetus. Traditional approaches often entailed radical excision of affected organs, frequently leading to significant morbidity and adverse reproductive consequences. However, advances in surgical technology, perioperative care, and a deeper understanding of disease mechanisms have enabled the development and adoption of organ-sparing strategies. These innovations are particularly relevant in the management of conditions such as placenta accreta spectrum, uterine fibroids, ovarian masses, and certain malignancies during pregnancy, where maternal health and future fertility are central concerns.
The global burden of obstetric and gynecologic conditions requiring surgical intervention remains substantial. Placenta accreta spectrum complicates approximately 1 in 533 pregnancies, with rising incidence due to increased cesarean delivery rates. Uterine fibroids affect up to 70% of women of reproductive age, and adnexal masses are detected in roughly 1–2% of pregnancies. Uterine fibroids arise from clonal expansion of smooth muscle cells, while ovarian masses can represent benign cysts, borderline tumors, or malignancies. Surgical intervention carries unique risks in pregnancy, such as hemorrhage and preterm labor, necessitating strategies that preserve uterine and ovarian function when feasible. Mechanism-based innovations, such as selective devascularization, uterine artery embolization, and targeted excision, underpin contemporary organ-sparing approaches.
Risk stratification is critical in patient selection for organ-sparing surgery. Key risk factors for placenta accreta include prior cesarean delivery, placenta previa, advanced maternal age, and assisted reproductive technologies. For uterine fibroids, risk is influenced by race, family history, and hormonal exposure. Ovarian masses are more prevalent in older reproductive-age women and in those with genetic predispositions. Identification of modifiable and non-modifiable risk factors informs preoperative planning and guides intraoperative decision-making to maximize organ preservation without jeopardizing patient safety.
Clinically, patients may present with abnormal uterine bleeding, pelvic pain, or incidental findings on prenatal imaging. Placenta accreta often manifests as unexplained third-trimester bleeding or difficulty with placental separation at delivery. Large fibroids can cause mass effect symptoms or complicate pregnancy. Ovarian masses may be asymptomatic or present with acute pain if torsion occurs. Recognizing these features allows for timely diagnosis and appropriate referral to specialized surgical teams skilled in organ-sparing techniques.
Accurate diagnosis is paramount for effective surgical planning. Contemporary practice employs high-resolution ultrasound and magnetic resonance imaging (MRI) for detailed assessment of placental invasion, myometrial integrity, and adnexal pathology. Biomarker assays, Doppler studies, and multidisciplinary case reviews further refine diagnostic accuracy. Early identification facilitates preoperative counseling regarding risks, benefits, and alternatives to radical surgery, enabling informed, patient-centered decision-making.
Organ-sparing surgical management is guided by disease type, severity, and gestational age. In placenta accreta spectrum, conservative measures such as leaving the placenta in situ with adjunctive methotrexate or uterine artery embolization have demonstrated success in select cases. Myomectomy during pregnancy remains controversial but may be performed in symptomatic, carefully selected patients with subserosal or pedunculated fibroids. Laparoscopic cystectomy or detorsion for benign ovarian masses has become increasingly feasible with advances in minimally invasive surgery. Perioperative strategies, including meticulous hemostasis, use of hemostatic agents, and real-time imaging, contribute to favorable outcomes.
Recent innovations include the adoption of robotic-assisted minimally invasive techniques, intraoperative navigation systems, and refined energy devices for precise tissue dissection and hemostasis. Novel hemostatic materials, such as fibrin sealants and resorbable meshes, further reduce perioperative blood loss. Pharmacologic adjuncts, including uterotonics and antifibrinolytics, have improved surgical safety profiles. Additionally, the growing role of multidisciplinary teams—including maternal-fetal medicine, gynecologic oncology, interventional radiology, and anesthesia—has enhanced perioperative planning and execution of complex organ-sparing procedures. Emerging data support the safety and efficacy of these innovations, though long-term maternal and reproductive outcomes remain areas of active investigation.
Professional societies, such as the American College of Obstetricians and Gynecologists (ACOG) and the Royal College of Obstetricians and Gynaecologists (RCOG), increasingly endorse organ-sparing approaches when clinically appropriate. Consensus guidelines recommend individualized assessment, multidisciplinary consultation, and shared decision-making. In cases of placenta accreta spectrum, conservative management may be considered for women desiring future fertility and in whom bleeding is controlled. For fibroids and ovarian masses, minimally invasive, organ-preserving techniques are preferred when malignancy is excluded and technical feasibility is established. Guidelines emphasize the importance of specialized surgical expertise and institutional readiness for potential complications.
Maternal organ-sparing surgical approaches represent a paradigm shift in women\"s health, offering improved preservation of reproductive and hormonal function with reduced morbidity. These innovations, grounded in pathophysiologic insight and supported by emerging evidence, require careful patient selection, advanced surgical skill, and adherence to evolving guidelines.
