This review synthesizes recent evidence on the impact of these pathways on QoL, evaluates underlying mechanisms, discusses clinical features and diagnosis, and explores current treatment modalities, emerging therapies, and relevant guideline recommendations. We highlight the importance of evidence-based personalization in perioperative medicine and its practical implications for improving both short- and long-term patient-centered outcomes.
Perioperative care has evolved from standardized protocols to increasingly personalized approaches, reflecting the heterogeneity of patient populations and surgical procedures. Enhanced Recovery After Surgery (ERAS) programs have demonstrated improved clinical outcomes and shorter hospital stays, but emerging evidence suggests that tailoring recovery pathways to individual patient characteristics yields further benefits, especially in terms of quality of life (QoL). This article examines the multifaceted impacts of personalized perioperative recovery pathways on QoL, integrating current guidelines, recent clinical trials, and expert consensus to provide actionable insights for practicing clinicians.
The burden of postoperative complications and impaired QoL is considerable, with millions of surgeries performed globally each year. Traditional perioperative management is associated with substantial variability in outcomes, including prolonged functional impairment and diminished QoL, especially in elderly or comorbid populations. Studies estimate that up to 30% of patients undergoing major surgery experience clinically significant declines in QoL in the months following the procedure. Personalized recovery pathways aim to address this burden by reducing complications, promoting faster functional recovery, and minimizing long-term morbidity.
The stress response to surgery triggers a cascade of neuroendocrine, inflammatory, and metabolic changes, which contribute to tissue injury, immunosuppression, and delayed recovery. Individual differences in genetic makeup, comorbidities, nutritional status, and psychological resilience influence the magnitude and duration of this response. Personalized perioperative pathways leverage patient-specific risk stratification, multimodal analgesia, and optimized nutrition to modulate these pathophysiological processes, thereby improving tissue healing, reducing catabolism, and facilitating earlier return to baseline function and QoL.

Assessment of postoperative QoL requires a multidimensional approach, utilizing both generic and condition-specific PROMs at baseline and at serial postoperative intervals. Comprehensive preoperative evaluation including geriatric assessment, frailty scoring, nutritional screening, and psychological evaluation enables the identification of modifiable factors that can be targeted in personalized recovery plans. Integration of electronic health records and digital health platforms further facilitates real-time monitoring and early identification of QoL impairments.
Personalized perioperative management encompasses prehabilitation (exercise, nutrition, and psychological preparation), minimally invasive surgical techniques, multimodal analgesia, early mobilization, and tailored discharge planning. Interventions are adapted based on patient-specific risk profiles, with an emphasis on interdisciplinary collaboration. For example, patients with high baseline anxiety may benefit from preoperative counseling, while those with frailty may require intensive physiotherapy and nutritional optimization. Postoperative follow-up is individualized to ensure early detection and management of complications or functional decline.
Recent advances include the integration of artificial intelligence (AI) and machine learning algorithms to predict patient risk and customize recovery protocols. Wearable devices and remote monitoring technologies enable continuous assessment of functional recovery and QoL in the home setting, allowing for timely intervention. Pharmacogenomics-guided analgesia and precision nutrition are emerging as promising strategies to further individualize perioperative care. Early data from randomized controlled trials suggest that these innovations may enhance patient satisfaction, reduce readmissions, and improve long-term QoL outcomes.
Major surgical and anesthesiology societies now advocate for the routine incorporation of personalized recovery pathways, with recommendations emphasizing risk stratification, shared decision-making, and the use of PROMs to guide care. The ERAS Society and American Society of Anesthesiologists highlight the importance of tailoring prehabilitation, analgesia, and post-discharge support based on patient characteristics. Guidelines also stress the need for robust communication between multidisciplinary team members to ensure cohesive, patient-centered management.
Personalized perioperative recovery pathways represent a paradigm shift in surgical care, with robust evidence supporting their efficacy in improving patient-reported quality of life. By addressing individual risk factors and tailoring interventions to patient needs, these pathways reduce complications, enhance functional recovery, and promote sustained well-being. Ongoing research into digital health, precision medicine, and patient-centered outcomes will further refine these approaches, ultimately delivering superior value and satisfaction for surgical patients worldwide.
