Renal functional preservation is a cornerstone of nephrology and broader healthcare practice, given the centrality of kidney health to overall physiological homeostasis and long-term health outcomes. This review synthesizes current evidence on epidemiology, pathophysiology, risk factors, clinical features, diagnostic strategies, therapeutic approaches, and emerging trends in renal functional preservation. Guideline-based recommendations and practical clinical insights are provided for healthcare professionals to optimize patient management and future health trajectories.
The kidneys play pivotal roles in maintaining fluid and electrolyte balance, regulating blood pressure, and excreting metabolic waste. Renal functional decline, whether acute or chronic, is associated with significant morbidity and mortality, impacting cardiovascular health, metabolic status, and quality of life. Preservation of renal function is therefore a critical objective in managing at-risk populations and in general clinical practice. The evolution of diagnostic tools, risk stratification models, and therapeutic strategies has provided new opportunities to delay progression of kidney disease and improve patient outcomes.
Chronic kidney disease (CKD) affects approximately 10% of the global population, with higher prevalence in aging populations and those with comorbidities such as diabetes and hypertension. End-stage renal disease (ESRD) poses a significant burden on healthcare systems, necessitating costly interventions like dialysis and transplantation. Acute kidney injury (AKI) remains a frequent complication in hospital settings, with substantial risk of progression to chronic renal impairment. Epidemiological data underscore the need for early identification and intervention to preserve renal function and reduce downstream complications.
Renal functional loss arises from complex and interrelated mechanisms, including glomerular hypertension, tubulointerstitial fibrosis, oxidative stress, and inflammation. In CKD, maladaptive responses to nephron loss lead to compensatory hyperfiltration and progressive scarring. AKI involves acute tubular injury, which, if unresolved, can trigger maladaptive repair and permanent nephron loss. Pathways such as the renin-angiotensin-aldosterone system (RAAS), transforming growth factor-beta (TGF-β), and oxidative stress mediators are central to disease progression and therapeutic targeting.
Major risk factors for renal functional decline include diabetes mellitus, hypertension, obesity, advancing age, genetic predisposition, and exposure to nephrotoxins. Cardiovascular disease, recurrent urinary tract infections, and autoimmune disorders such as lupus nephritis further increase susceptibility. Socioeconomic determinants and disparities in healthcare access also influence risk trajectories. Identification and mitigation of modifiable risk factors are essential in clinical practice to preserve renal function.
Early stages of renal dysfunction are often asymptomatic and may be detected only through laboratory abnormalities such as elevated serum creatinine or reduced estimated glomerular filtration rate (eGFR). As renal function declines, patients may develop hypertension, volume overload, electrolyte disturbances, and symptoms of uremia, including fatigue, anorexia, nausea, and pruritus. Recognition of subtle clinical cues is vital for timely diagnosis and intervention.
Diagnosis of renal dysfunction relies on a combination of clinical assessment, laboratory investigations, and imaging. eGFR, based on serum creatinine or cystatin C, is the cornerstone of renal function assessment. Proteinuria and albuminuria are key prognostic markers, often quantified by urine albumin-to-creatinine ratio (ACR). Imaging modalities such as renal ultrasound can identify structural abnormalities, while renal biopsy remains the gold standard in select cases for definitive diagnosis and prognostication.
Management strategies center on addressing underlying etiologies, controlling blood pressure, and minimizing exposure to nephrotoxins. RAAS inhibition with angiotensin-converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) is foundational in reducing proteinuria and slowing CKD progression. Glycemic control in diabetes, lipid management, and lifestyle interventions such as dietary sodium restriction and weight optimization play critical roles. Multidisciplinary care, patient education, and early nephrology referral are associated with improved outcomes.
Recent advances in renal preservation include the development of sodium-glucose co-transporter 2 (SGLT2) inhibitors, which confer renal and cardiovascular protection in diabetic and non-diabetic CKD. Non-steroidal mineralocorticoid receptor antagonists (ns-MRAs), such as finerenone, have shown efficacy in reducing albuminuria and slowing progression. Novel biomarkers, regenerative therapies, and artificial intelligence-driven risk prediction models are under investigation. Personalized medicine approaches, leveraging genetic and molecular profiling, are poised to revolutionize renal care.
Current guidelines from the Kidney Disease: Improving Global Outcomes (KDIGO) organization emphasize early detection, risk stratification, and evidence-based interventions tailored to disease stage and etiology. Blood pressure targets, glycemic thresholds, and indications for initiating pharmacotherapy are regularly updated based on emerging trial data. Guideline-based multidisciplinary care, incorporating dietitians, pharmacists, and primary care, is recommended to optimize renal and overall health trajectories.
Renal functional preservation is integral to improving long-term health outcomes, reducing healthcare costs, and enhancing quality of life for patients with or at risk for kidney disease. A comprehensive, guideline-driven, and patient-centered approach encompassing risk factor modification, early diagnosis, targeted therapy, and adoption of novel interventions is essential. Ongoing research and innovation are expanding therapeutic horizons and offering renewed hope for the future of renal care.
