Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, remains a significant global health issue. Despite advancements in diagnostic methods and therapeutic strategies, the disease continues to pose challenges in clinical practice. This article aims to provide an updated approach to the diagnosis and management of enteric fever.
Early diagnosis of enteric fever is crucial for effective management. Traditional culture-based methods are being replaced by rapid diagnostic tests (RDTs), offering higher sensitivity and specificity. However, the gold standard remains blood culture, which can detect the disease in the early stages. Molecular methods, such as Polymerase Chain Reaction (PCR), offer a promising alternative, providing rapid and accurate results. Serological tests like Widal test have limited value due to high false-positive rates.
Antibiotic therapy is the mainstay of enteric fever treatment. The choice of antibiotics is guided by local susceptibility patterns. Fluoroquinolones, such as ciprofloxacin, were widely used, but increasing resistance has led to a shift towards newer agents like azithromycin and third-generation cephalosporins. However, the emergence of extensively drug-resistant (XDR) strains necessitates the prudent use of antibiotics. Adjunctive therapies, including hydration and fever management, remain essential components of the treatment protocol.
Vaccination plays a crucial role in enteric fever prevention. Two vaccines, Vi polysaccharide vaccine and Ty21a, are currently available. However, their effectiveness varies, and neither provides long-term protection. Hence, improvements in sanitation and hygiene, along with targeted vaccination campaigns, are vital for disease control.
Enteric fever continues to evolve with changing epidemiological trends and antibiotic resistance patterns. Therefore, it is imperative to stay updated with the latest diagnostic methods and treatment strategies. A holistic approach encompassing prompt diagnosis, appropriate antibiotic therapy, and preventive measures can significantly improve patient outcomes in clinical practice.
