Vertigo, a sensation of feeling off balance, is a prevalent symptom often encountered in clinical practice. It is a complex condition that can be challenging to diagnose and manage due to its multifactorial nature. This article aims to provide a comprehensive clinical approach to vertigo, facilitating accurate diagnosis and effective management.
Vertigo is primarily a symptom of various diseases rather than an illness itself. It can be peripheral, central, or systemic in origin. The most common cause is benign paroxysmal positional vertigo (BPPV), followed by Meniere's disease and vestibular neuritis. Understanding the pathophysiology of these conditions is critical for determining the appropriate diagnostic tests and treatment options.
The clinical evaluation of vertigo begins with a thorough history and physical examination. The history should focus on the onset, duration, frequency, and accompanying symptoms. The physical examination should include a detailed neurotological examination, including tests for nystagmus, head impulse test, and Romberg's sign. Additional diagnostic tests may include audiometry, vestibular function tests, and neuroimaging.
Management of vertigo depends on the underlying cause. In BPPV, canalith repositioning maneuvers are the first-line treatment. Meniere's disease may require dietary modifications, diuretics, and vestibular suppressants. Vestibular rehabilitation therapy is beneficial for most types of vertigo. Pharmacological management includes medications such as meclizine, prochlorperazine, and diazepam. In refractory cases, surgical interventions may be considered.
Vertigo is a complex symptom with a myriad of potential underlying causes. A comprehensive clinical approach, including a detailed history, physical examination, and appropriate diagnostic tests, can aid in accurate diagnosis and effective management. Keeping abreast of the latest research and advancements in the field is essential for providing optimal care to patients with vertigo.
