Antimicrobial stewardship programs (ASPs) are vital to curbing the escalating issue of antimicrobial resistance (AMR), a global health crisis. By promoting the appropriate use of antimicrobials, ASPs help to improve patient outcomes, reduce microbial resistance, and decrease the spread of infections caused by multidrug-resistant organisms. Here, we outline a strategic approach for hospitals to implement effective ASPs.
Successful ASPs require a multidisciplinary team approach. This team should ideally include an infectious disease physician, a clinical pharmacist with infectious disease training, clinical microbiologists, information system specialists, an infection control professional, and hospital administration. This diverse team can collaboratively develop, implement, and monitor ASP interventions.
ASPs should be tailored to the specific needs and resources of the hospital. Interventions may include prospective audit and feedback, formulary restriction, and preauthorization requirements. The interventions should be evidence-based, patient-centered, and adaptable to changes in the local microbial ecosystem and resistance patterns.
Regular measurement and feedback of both process and outcome measures are crucial for the success of ASPs. Process measures include antimicrobial use metrics, while outcome measures include rates of Clostridioides difficile infection and antibiotic resistance. This data can help identify areas of success and areas needing improvement.
Education and training are essential components of ASPs. Regular education sessions can ensure that all healthcare providers are up-to-date on the principles of antimicrobial stewardship and the specific interventions being used at their hospital.
In conclusion, the implementation of effective ASPs in hospitals requires a strategic, multidisciplinary approach. By building a diverse team, developing tailored interventions, regularly measuring outcomes, and prioritizing education, hospitals can significantly contribute to the global effort to combat antimicrobial resistance.
