Frailty, a multidimensional syndrome characterized by decreased physiological reserve and resilience, has emerged as a critical predictor of adverse postoperative outcomes. This review synthesizes current evidence on the importance of frailty screening prior to elective surgery, highlighting epidemiological data, pathophysiological mechanisms, risk factors, clinical features, diagnostic tools, management strategies, and recent advances. It emphasizes guideline-based recommendations and practical implications for perioperative care, aiming to inform clinicians on optimizing surgical outcomes among frail patients.
Elective surgical procedures in older adults are increasingly common due to population aging and advances in surgical care. However, the presence of frailty significantly elevates the risk of complications, prolonged hospitalization, functional decline, and mortality. Early identification of frailty through systematic screening is essential for risk stratification, individualized care planning, and improved perioperative outcomes. This review explores the clinical and scientific rationale for integrating frailty assessment into preoperative pathways, with a focus on evidence-based recommendations and practical strategies for implementation.
The prevalence of frailty among older surgical candidates ranges from 10% to 50%, depending on the definition and screening tool used. Frailty is associated with a two- to fourfold increase in postoperative complications, including delirium, infections, functional decline, and mortality. Recent multicenter cohort studies and meta-analyses demonstrate that frail patients experience higher rates of readmission, longer hospital stays, and increased use of intensive care resources. The expanding surgical population of older adults underlines the urgent need for effective frailty screening to optimize surgical risk assessment and resource allocation.
Frailty results from cumulative deficits in multiple physiological systems, including musculoskeletal, cardiovascular, immune, and neuroendocrine pathways. Mechanistically, it reflects a diminished ability to maintain homeostasis under stress, such as surgery. Chronic inflammation, hormonal dysregulation, sarcopenia, and impaired energy metabolism contribute to the frailty phenotype. The surgical stress response can overwhelm compromised physiologic reserves in frail patients, precipitating adverse outcomes through mechanisms like impaired wound healing, altered drug metabolism, and increased susceptibility to delirium and infections.
Common risk factors for frailty include advanced age, polypharmacy, comorbid conditions (e.g., cardiovascular disease, diabetes, chronic kidney disease), nutritional deficits, cognitive impairment, and physical inactivity. Social determinants of health, such as isolation and socioeconomic disadvantage, further exacerbate frailty risk. Preoperative identification of these factors aids in targeting high-risk individuals for comprehensive geriatric assessment and tailored perioperative interventions.
Frailty manifests clinically as unintentional weight loss, weakness, slowed gait speed, exhaustion, and reduced physical activity. Additional features may include cognitive decline, decreased mobility, and increased vulnerability to stressors. Objective assessment tools, such as the Fried Frailty Phenotype and the Clinical Frailty Scale, quantify these features to stratify surgical risk. Recognizing these manifestations enables early multidisciplinary intervention to mitigate postoperative morbidity.
Several validated instruments are available for frailty screening in the surgical setting. The Fried Criteria assess five domains: weight loss, exhaustion, physical activity, gait speed, and grip strength. The Clinical Frailty Scale provides a global assessment based on clinical judgment. The Edmonton Frail Scale and the Frailty Index incorporate cognitive and psychosocial domains. Integrating frailty screening into preoperative evaluation protocols ensures systematic identification of at-risk patients and supports shared decision-making regarding surgical candidacy and perioperative management.
Management of frailty in the perioperative period involves a comprehensive, multidisciplinary approach. Prehabilitation, including exercise, nutritional optimization, and cognitive training, has shown promise in improving functional status and surgical outcomes. Medication review and deprescribing reduce polypharmacy risks. Enhanced Recovery After Surgery (ERAS) protocols, geriatric co-management, and tailored anesthesia plans further mitigate complications. Postoperative care should focus on early mobilization, delirium prevention, and rehabilitation to restore baseline function and minimize long-term disability.
Recent advances in frailty research include the development of rapid, user-friendly screening tools suitable for busy surgical clinics and the implementation of digital health platforms for remote monitoring. Biomarker research, exploring inflammatory and metabolic signatures of frailty, holds promise for objective risk stratification. Emerging evidence supports the effectiveness of multidisciplinary prehabilitation programs and telemedicine in delivering individualized preoperative care to frail patients. Ongoing trials are evaluating novel interventions targeting sarcopenia and cognitive resilience, aiming to further reduce perioperative risk in this vulnerable population.
International guidelines, including those from the American College of Surgeons and the British Geriatrics Society, endorse routine frailty screening for older adults undergoing elective surgery. Recommendations emphasize the use of validated assessment tools, incorporation of findings into risk discussions, and development of individualized perioperative care plans. Multidisciplinary collaboration among surgeons, anesthesiologists, geriatricians, and allied health professionals is essential for optimal implementation of frailty assessment and management strategies.
Frailty screening before elective surgery is a critical component of modern perioperative care for older adults. It enables early identification of high-risk patients, informs shared decision-making, and facilitates multidisciplinary interventions that can reduce postoperative complications and improve functional outcomes. Incorporating evidence-based frailty assessment into routine surgical practice aligns with current guidelines and represents a paradigm shift towards precision risk stratification and personalized perioperative care.
