The article aims to provide clinicians with a comprehensive understanding of the mechanisms and interventions that foster sustainable identity change, thereby improving clinical outcomes for individuals with SUD.
The chronic and relapsing nature of addiction imposes profound changes on an individual's identity, relationships, and life trajectory. Beyond the attainment of abstinence, long-term recovery necessitates a process of identity reconstruction a shift from an "addict" identity to one aligned with health, autonomy, and social integration. This transformation is influenced by neurobiological adaptations, psychosocial dynamics, and environmental factors. Increasingly, evidence-based interventions recognize identity work as pivotal for enduring recovery and relapse prevention. The present review explores the multidimensional process of identity reconstruction during long-term recovery, integrating current scientific literature, clinical practice guidelines, and expert consensus to inform healthcare professionals on best practices for supporting this critical aspect of recovery.
Substance use disorders affect over 35 million people globally, with relapse rates estimated at 40-60% within the first year of abstinence. Chronic relapse and recidivism are closely tied to persistent maladaptive identity constructs, social stigma, and lack of supportive environments conducive to identity change. Epidemiological studies demonstrate that individuals who successfully transition to a recovery-oriented identity exhibit lower relapse rates, improved psychological well-being, and enhanced social functioning. The burden of SUD extends beyond physical health, encompassing occupational, legal, and interpersonal domains further underscoring the necessity of holistic approaches that address identity reconstruction as a determinant of long-term recovery success.
Addiction fundamentally alters neural circuits governing reward, motivation, learning, and self-regulation.
