Enteric fever, commonly known as typhoid fever, presents a significant global health challenge, particularly in low-resource settings. Early diagnosis and effective management are crucial to reduce morbidity and mortality. This article provides an integrated approach to the diagnosis and management of enteric fever.
Diagnosis of enteric fever is primarily based on clinical suspicion, backed up by microbiological confirmation. Blood culture remains the gold standard, despite its limitations in sensitivity. Molecular methods like PCR have shown promise, but are not widely available in resource-limited settings. Serological tests, despite their lower sensitivity and specificity, are often used due to their cost-effectiveness and ease of use.
The mainstay of treatment for enteric fever is antimicrobial therapy. Choice of antimicrobial should be guided by local susceptibility patterns. Fluoroquinolones, third-generation cephalosporins, and azithromycin are commonly used. Resistance to these agents is a growing concern, necessitating regular antimicrobial susceptibility testing. Adjunctive care, including hydration and symptom management, is also important.
Prevention of enteric fever relies on improving sanitation and access to clean water. Vaccination can also play a role, particularly in endemic areas. The WHO recommends the typhoid conjugate vaccine for children over six months in endemic areas. However, vaccination should not replace efforts to improve water, sanitation, and hygiene.
Enteric fever remains a significant health burden, particularly in resource-limited settings. An integrated approach to diagnosis and management, incorporating clinical suspicion, microbiological confirmation, effective antimicrobial therapy, and preventive measures, can significantly reduce this burden. As healthcare professionals, we have a pivotal role in implementing these strategies and advocating for improved access to clean water and sanitation.
