This review synthesizes current evidence, explores the epidemiology and pathophysiology underlying complex chronic conditions in aging populations, and examines risk factors, clinical features, diagnostic challenges, and contemporary management strategies. It further discusses recent advances, guideline recommendations, and the practical implications of fostering age-friendly environments within healthcare systems, offering actionable insights for clinicians and stakeholders.
\nAs global populations age, the burden of complex chronic diseases escalates, presenting multifaceted challenges to healthcare professionals. Traditional care models often fall short in addressing the needs of older adults with multimorbidity, polypharmacy, and functional impairments. Age-friendly communities, encompassing both clinical and social determinants, are vital in delivering holistic care.
