The prevalence of patients with multiple chronic diseases referred to as multimorbidity has risen sharply with global aging and improvements in chronic disease survival. Clinicians must balance competing priorities and limited resources, often in the context of fragmented guidelines. Case-based clinical prioritization refers to the systematic process of ranking clinical issues and interventions based on severity, modifiability, patient preferences, and potential impact on quality of life. This approach is increasingly recognized as integral to high-quality, personalized care for complex patients.
Multimorbidity affects over 30% of adults in developed nations, with prevalence exceeding 65% in those over 65 years. The disease burden is disproportionately high among socioeconomically disadvantaged populations. Common clusters include diabetes, hypertension, cardiovascular disease, chronic kidney disease, and depression. The co-occurrence of these conditions is associated with increased healthcare utilization, higher mortality, and diminished functional status. Major guidelines, including those from the American Geriatrics Society and NICE, advocate for individualized, case-based prioritization rather than rigid adherence to disease-specific protocols in patients with multimorbidity. Key recommendations emphasize regular medication review, deprescribing, assessment of life expectancy and functional status, and shared decision-making. Multidisciplinary care coordination and advanced care planning are also strongly advised, particularly for patients with limited life expectancy or frailty.
