Abstract

Exercise-induced respiratory disorders (EIRDs) encompass a spectrum of airway and parenchymal pathologies that are triggered or exacerbated by physical exertion. These conditions, with exercise-induced bronchoconstriction (EIB) and exercise-induced laryngeal obstruction (EILO) as primary entities, are frequently under-recognized among athletes and physically active individuals, often leading to misdiagnosis and suboptimal management. This review provides a comprehensive analysis of epidemiology, pathophysiology, clinical presentation, diagnostic approaches, and evidence-based management strategies, supported by recent clinical guidelines and emerging research. Special attention is given to disease burden, risk stratification, mechanism-based interventions, and practical clinical implications for healthcare professionals involved in the care of affected populations.

Introduction

Exercise-induced respiratory disorders represent a significant clinical challenge, particularly in athletic and pediatric populations. These disorders are characterized by transient or persistent respiratory symptoms provoked by physical activity, impacting exercise tolerance, performance, and overall quality of life. The broad clinical spectrum includes conditions such as EIB, EILO, and less commonly, exercise-induced pulmonary hemorrhage (EIPH) and exercise-induced asthma (EIA). Accurate recognition, differentiation, and management are essential, as misdiagnosis may result in inappropriate therapies and continued morbidity. This review synthesizes current literature to inform best practices in clinical evaluation and management.

Epidemiology / Disease Burden

EIRDs are highly prevalent, with EIB affecting approximately 5-20% of the general population and up to 90% of individuals with underlying asthma. Among elite athletes, prevalence rates for EIB range from 10% to 50%, with higher rates observed in endurance sports (e.g., long-distance running, swimming, cross-country skiing). EILO is increasingly recognized, particularly in adolescent females, with prevalence estimates of 5-15% in populations referred for exercise-induced dyspnea. The public health burden is significant due to reduced physical activity, impaired athletic performance, healthcare utilization, and psychosocial impact. Despite this, EIRDs remain underdiagnosed, highlighting the need for heightened clinical suspicion and systematic evaluation.

Pathophysiology

The pathophysiological mechanisms underlying EIRDs are multifactorial and condition-specific. EIB is primarily attributed to airway dehydration and cooling during high minute ventilation, leading to hyperosmolarity and subsequent release of inflammatory mediators (e.g., histamine, leukotrienes) from airway mast cells. This results in bronchial smooth muscle contraction and increased airway resistance. EILO, in contrast, involves paradoxical vocal cord or supraglottic closure during inspiration, likely precipitated by heightened laryngeal sensitivity and neuromuscular dysfunction. EIPH is predominantly seen in high-intensity athletes due to increased pulmonary capillary pressure and stress failure of the alveolar-capillary barrier. The interplay between genetic predisposition, environmental exposures (e.g., cold air, pollutants), and atopy further modulates disease expression.

Risk Factors

Multiple risk factors contribute to EIRDs, including pre-existing asthma, atopic diathesis, family history of respiratory disease, exposure to environmental irritants (chlorine, cold air, allergens), high-intensity or endurance training, and inadequate warm-up routines. EILO is more common in adolescent females, possibly due to anatomical and hormonal influences. Poorly controlled allergic rhinitis, gastroesophageal reflux, and psychological stress have also been implicated. Identification of individualized risk profiles is critical for both prevention and targeted management.

Clinical Features

EIRDs present with exertional dyspnea, cough, wheezing, chest tightness, and occasionally stridor or hoarseness. The timing and character of symptoms are diagnostically informative; EIB typically manifests 5-15 minutes post-exercise, whereas EILO often presents with abrupt inspiratory difficulty during peak exercise, resolving with cessation of activity. EIPH may present with hemoptysis in rare cases. Symptom overlap with other cardiorespiratory and psychological conditions necessitates careful clinical assessment and differential diagnosis.

Diagnosis

Definitive diagnosis of EIRDs requires objective testing in addition to clinical history. Spirometry with pre- and post-exercise measurements remains the gold standard for EIB, with a ≥10% decline in FEV1 post-exercise considered diagnostic. Bronchoprovocation tests using eucapnic voluntary hyperpnea or inhaled mannitol may improve sensitivity. Continuous laryngoscopy during exercise (CLE test) is the preferred modality for diagnosing EILO, allowing visualization of dynamic laryngeal obstruction. Ancillary investigations may include allergen testing, exhaled nitric oxide measurement, and pulse oximetry. Accurate diagnosis is essential to guide therapy and avoid unnecessary pharmacologic interventions.

Treatment & Management

Management of EIRDs is multifaceted and individualized. For EIB, the cornerstone of therapy includes inhaled short-acting beta-agonists (SABAs) administered prior to exercise, with inhaled corticosteroids (ICS) indicated for persistent cases or coexisting asthma. Leukotriene receptor antagonists and mast cell stabilizers may provide adjunctive benefit. Non-pharmacologic strategies include structured warm-up protocols, avoidance of triggers, and environmental modifications. EILO management focuses on laryngeal control therapy, with speech-language pathology interventions demonstrating high efficacy. Rarely, surgical procedures may be required for severe, refractory EILO. Multidisciplinary collaboration is essential for optimal outcomes.

Recent Advances / Emerging Therapies

Recent advances have expanded therapeutic options and diagnostic precision. Novel inhaled anti-inflammatory agents, biologics targeting type 2 inflammation, and personalized medicine approaches are under investigation for EIB, particularly in severe or corticosteroid-resistant cases. Portable laryngoscopy and real-time assessment tools have improved detection of EILO. Telemedicine and remote monitoring platforms are increasingly utilized for symptom tracking and adherence support. Ongoing research into the genetic and molecular underpinnings of EIRDs may yield novel biomarkers for risk stratification and early intervention.

Guideline Recommendations

Current international guidelines, including those from the Global Initiative for Asthma (GINA) and American Thoracic Society (ATS), emphasize the importance of objective testing in EIRD diagnosis, individualized pharmacologic and non-pharmacologic management, and ongoing monitoring of therapeutic response. For EIB, pre-exercise SABA use and maintenance ICS in persistent cases are strongly recommended. EILO guidelines advocate for laryngeal control therapy as first-line management, with surgical intervention reserved for selected cases. Regular re-evaluation and patient education are critical components of care.

Exercise-induced respiratory disorders are prevalent, clinically impactful conditions requiring a high index of suspicion and evidence-based management. Advances in diagnostic techniques and personalized therapies have improved outcomes for affected individuals, although continued research is warranted. Healthcare professionals should maintain awareness of EIRD phenotypes, adhere to guideline-based management, and engage in multidisciplinary collaboration to optimize patient care and promote safe participation in physical activity.
