In males, semen analysis remains the cornerstone, complemented by hormonal profiling (FSH, LH, testosterone, inhibin B). Timely and accurate diagnosis facilitates early referral to reproductive specialists and enables risk-adapted fertility preservation planning.

Treatment & Management

Fertility preservation encompasses a spectrum of interventions tailored to patient age, cancer type, and treatment urgency. Established options in females include embryo and oocyte cryopreservation, ovarian tissue freezing, and, less commonly, ovarian transposition. For males, sperm cryopreservation is the standard of care, with testicular tissue freezing reserved for prepubertal boys. Adjunctive measures, such as gonadotropin-releasing hormone (GnRH) analogs, have demonstrated variable efficacy in ovarian protection. Multidisciplinary collaboration involving oncologists, reproductive endocrinologists, and psychosocial support teams is paramount for optimizing outcomes and addressing the complex needs of young survivors.

Recent Advances / Emerging Therapies

Recent years have witnessed significant progress in fertility preservation technologies. Vitrification techniques have improved oocyte and embryo survival rates, while advances in ovarian tissue transplantation have enabled successful pregnancies post-cancer treatment. Experimental modalities, including in vitro maturation of oocytes and artificial gametogenesis, hold promise for future applications. Moreover, the integration of fertility preservation discussions into electronic health records and survivorship care plans has enhanced counseling rates and patient satisfaction. Guidelines emphasize individualized risk assessment, timely referral to fertility specialists, and informed shared decision-making. Documentation of counseling, respect for patient autonomy, and consideration of ethical, cultural, and financial factors are integral components of best practice. Multidisciplinary care models are encouraged to ensure comprehensive support throughout the cancer trajectory and into survivorship.
