This review synthesizes recent advances in epidemiology, pathophysiology, clinical presentation, diagnosis, and management, highlighting the clinical implications and guideline recommendations for effective care. The dual burden of muscle loss and excess fat confers increased risks for disability, metabolic dysfunction, and mortality in older adults, necessitating multidisciplinary approaches and novel therapeutic strategies. Understanding the nuances of sarcopenic obesity is essential for clinicians to optimize outcomes in the aging population.

Introduction

Sarcopenic obesity represents an intersection of two major public health concerns: sarcopenia, defined by progressive loss of muscle mass and function, and obesity, characterized by excessive fat accumulation. In the geriatric population, this synergy poses unique diagnostic and therapeutic challenges. The condition is increasingly recognized as a driver of frailty, impaired mobility, and chronic disease, yet remains underdiagnosed and undertreated. Recent research underscores the need for precise definitions, reliable diagnostic criteria, and targeted interventions, as traditional obesity or sarcopenia management strategies may not suffice for this complex phenotype.

Diagnosis

Diagnosing sarcopenic obesity requires accurate quantification of both muscle mass and adiposity. Gold-standard methods include dual-energy X-ray absorptiometry (DXA) and bioelectrical impedance analysis (BIA) for body composition, combined with functional assessments such as handgrip strength and gait speed. Consensus definitions from the European Working Group on Sarcopenia in Older People (EWGSOP) and the Asian Working Group for Sarcopenia (AWGS) recommend specific cut-offs for muscle mass and function, while obesity is typically defined by body fat percentage or waist circumference. Recent guidelines advocate for the integration of physical performance measures to enhance diagnostic precision.

Treatment & Management

Management of sarcopenic obesity is multifaceted, involving lifestyle modification, physical rehabilitation, nutritional support, and treatment of underlying comorbidities. Resistance and aerobic exercise programs are central, aiming to improve muscle strength and reduce fat mass. Nutritional interventions emphasize adequate protein intake, caloric balance, and micronutrient optimization. Pharmacologic therapies, such as anabolic agents or anti-obesity medications, are under investigation but are not yet routinely recommended. Comprehensive geriatric assessment and individualized care plans are critical to address the heterogeneity of patient needs and risks.

Recent Advances / Emerging Therapies

Recent advances in understanding the molecular mechanisms of sarcopenic obesity have spurred development of novel therapeutics. Myostatin inhibitors, selective androgen receptor modulators (SARMs), and anti-inflammatory agents show promise in early-phase trials. Nutraceuticals such as omega-3 fatty acids and vitamin D supplementation are also being explored for their potential to modulate muscle and fat metabolism. International guidelines highlight the importance of routine screening for sarcopenic obesity in older adults with obesity, frailty, or functional decline. Multidisciplinary management, incorporating physical therapy, nutritional counseling, and medical optimization, is recommended. Regular monitoring of muscle strength, body composition, and functional status is essential for tailoring interventions. Guidelines also emphasize the need for individualized goals, considering patient preferences, comorbidities, and risk of adverse outcomes.

Sarcopenic obesity is a complex, underrecognized syndrome with profound implications for geriatric health. Early identification, comprehensive assessment, and integrated management strategies are paramount to mitigate risks and improve quality of life in affected individuals. Ongoing research and emerging therapies hold promise for more effective interventions, but translation into clinical practice requires continued interdisciplinary collaboration and patient-centered care.
