Furthermore, leveraging technology can facilitate the implementation by enhancing patient-provider communication and information sharing.
As the healthcare landscape continues to evolve, PCC models must adapt to meet emerging needs. Future research should focus on developing innovative strategies for implementing PCC in diverse healthcare settings, exploring the role of technology in facilitating PCC, and examining the long-term effects of PCC on health outcomes and healthcare costs.
In conclusion, PCC models hold significant promise for improving healthcare outcomes and patient satisfaction. However, their successful implementation requires overcoming substantial challenges. Through continued research and the adoption of innovative strategies, the full potential of PCC in modern healthcare practice can be realized.
