Pulmonary hypertension (PH) is a complex and often misunderstood disease. This article aims to clarify the intricacies of PH, and provide medical professionals with a succinct yet comprehensive understanding of this condition.
PH is characterized by increased pulmonary vascular resistance leading to right ventricular failure. The etiology is multifaceted, often related to lung diseases, heart diseases, or thromboembolic events. The pathophysiology involves endothelial dysfunction, vascular remodeling, and inflammation.
Patients typically present with non-specific symptoms such as dyspnea, fatigue, chest pain, and syncope. The insidious onset and vague symptomatology often lead to delayed diagnosis.
Diagnosis requires a high index of suspicion. Echocardiography is the initial investigation of choice, followed by right heart catheterization for definitive diagnosis. Other tests include pulmonary function tests, chest radiography, and CT pulmonary angiography.
Treatment focuses on the underlying cause, symptom management, and slowing disease progression. Therapies include vasodilators, endothelin receptor antagonists, phosphodiesterase-5 inhibitors, and guanylate cyclase stimulators. Severe cases may require lung transplantation.
The prognosis of PH is generally poor due to late diagnosis and limited treatment options. However, early diagnosis and aggressive management can significantly improve survival rates.
In conclusion, PH is a complex disease requiring astute clinical acumen for diagnosis and management. As medical professionals, it is imperative to stay updated with the latest knowledge and developments in this field to provide optimal patient care.
