The first step in evaluating elevated liver enzymes is to confirm the abnormality with repeat testing. Transient elevations can occur due to various reasons, including alcohol intake or medication use. The pattern of liver enzyme elevation, whether hepatocellular or cholestatic, can provide clues to the underlying cause.
Once persistent elevation is confirmed, further investigations should be guided by the patient's history, physical examination, and initial laboratory results. Common causes such as viral hepatitis, alcohol-related liver disease, and nonalcoholic fatty liver disease should be considered. Less common causes like autoimmune or genetic disorders may also be relevant in certain patients.
Treatment should be targeted at the underlying cause of the enzyme elevation. For instance, patients with alcohol-related liver disease should be counseled on abstaining from alcohol, while those with nonalcoholic fatty liver disease may benefit from weight loss and improved glycemic control. In cases where a specific cause cannot be identified, a watchful waiting approach may be appropriate.
Referral to a hepatologist or gastroenterologist should be considered in patients with persistently elevated liver enzymes despite initial management, those with advanced liver disease, or when the diagnosis is uncertain. These specialists can provide further diagnostic tests and treatments, including liver biopsy if needed.
Elevated liver enzymes are a common clinical problem that requires a systematic approach for evaluation and management. By considering the patient's history, physical examination, and initial laboratory results, clinicians can identify the most likely causes and initiate appropriate treatment strategies. Referral to a specialist should be considered in complex cases.
