FNA biopsy is the most reliable method to differentiate benign from malignant nodules. The Bethesda System for Reporting Thyroid Cytopathology is the universally accepted classification system. It provides a standardized approach, guiding management based on the risk of malignancy associated with each category.
In cases where cytology results are indeterminate, molecular testing can be used to further stratify the risk of malignancy. These tests analyze the presence of genetic alterations associated with thyroid cancer, aiding in the decision to proceed with surgery.
Management strategies range from active surveillance for low-risk nodules to surgery for high-risk nodules. The choice of treatment should be individualized, taking into account the patient's preferences, comorbidities, and overall risk profile.
