Key mechanisms underlying addiction, risk factors, diagnostic strategies, and evidence-based interventions are explored, with a focus on recent advances and guideline-directed practices that promote long-term recovery, functional restoration, and improved life satisfaction.
Addiction, defined as a chronic, relapsing disorder characterized by compulsive substance use despite harmful consequences, is a significant contributor to global morbidity and mortality. While traditional care models primarily addressed acute withdrawal and stabilization, contemporary addiction medicine increasingly recognizes the importance of long-term recovery and quality of life (QoL) as central therapeutic goals. Sustainable recovery encompasses not just abstinence, but also psychological well-being, social integration, and functional improvement. Community reintegration returning individuals to productive societal roles has emerged as a critical determinant of enduring recovery and holistic rehabilitation. This review aims to provide clinicians and healthcare professionals with a comprehensive, evidence-based overview of sustainable recovery strategies, integrating recent guideline recommendations and emerging therapies.
Globally, substance use disorders (SUDs) affect over 35 million people, with a disproportionate burden observed in marginalized and underserved populations. The World Health Organization reports that SUDs contribute significantly to disability-adjusted life years (DALYs), with opioid, alcohol, and stimulant use as leading contributors. Relapse rates remain high, with estimates ranging from 40-60% within the first year post-treatment. Socioeconomic consequences include loss of productivity, increased healthcare utilization, and heightened criminal justice involvement. Beyond individual suffering, addiction imposes a substantial economic burden estimated at over $740 billion annually in the United States alone underscoring the urgency for sustainable solutions that extend beyond acute care.
