Chronic diseases such as diabetes mellitus, hypertension, and heart failure require long-term, multifaceted management strategies. Adherence to pharmacotherapy is a cornerstone for achieving optimal clinical outcomes, yet suboptimal adherence remains a pervasive challenge. Emerging evidence highlights the roles of medication self-efficacy—the patient’s belief in their ability to manage medications successfully—and therapeutic empowerment—the process of acquiring knowledge, skills, and confidence to actively participate in care decisions—as determinants of adherence and QoL. Understanding and integrating these concepts into clinical practice is essential for advancing patient-centered outcomes and optimizing healthcare resource utilization.
