Acute chest pain (ACP) is a common symptom that often presents a diagnostic challenge to clinicians due to its diverse etiologies. The high-risk nature of conditions like acute coronary syndrome (ACS) necessitates an efficient and accurate evaluation process.
The initial assessment should be aimed at identifying life-threatening conditions. This includes obtaining a thorough history, performing a physical examination and ECG. The history should focus on the characteristics of the pain, associated symptoms, risk factors, and previous cardiac history. The physical examination should include vital signs, cardiac and lung auscultation, and examination for peripheral signs of cardiac disease.
Following the initial assessment, diagnostic testing should be considered. The choice of tests should be guided by the pre-test probability of ACS. For patients with a high pre-test probability, immediate invasive strategies should be considered. For patients with an intermediate or low pre-test probability, non-invasive testing such as coronary CT angiography or stress testing may be appropriate. Biomarkers, particularly troponins, can provide additional diagnostic information and should be measured in all patients.
Clinical decision rules like the HEART score can aid in risk stratification and guide further management. These tools integrate clinical, ECG, and biomarker data to estimate the likelihood of major adverse cardiac events. Their use can improve efficiency and reduce unnecessary hospital admissions.
Deciphering the enigma of acute chest pain requires a comprehensive approach that combines a detailed history, physical examination, appropriate diagnostic testing, and the use of validated clinical decision rules. This approach allows for efficient risk stratification, accurate diagnosis, and appropriate management, thereby improving patient outcomes and healthcare resource utilization.
