Multimorbidity, defined as the coexistence of two or more chronic conditions in an individual, is increasingly prevalent among older adults, presenting complex challenges for clinicians. This review synthesizes current evidence on the epidemiology, pathophysiology, risk factors, clinical features, and diagnostic approach to multimorbidity, with a particular focus on integrated care models. Recent research, guideline recommendations, and emerging therapies are discussed, highlighting the importance of patient-centered, interdisciplinary strategies to optimize outcomes and minimize treatment burden in this vulnerable population.
Older adults are disproportionately affected by multimorbidity, a phenomenon that complicates medical management and adversely affects quality of life, healthcare utilization, and mortality. Traditional single-disease focused care models are ill-equipped to address the complex interplay between multiple chronic conditions. Integrated care, characterized by coordinated, multidisciplinary interventions tailored to individual needs, has emerged as a promising approach. This article provides an in-depth review of the scientific and clinical foundations of multimorbidity management in older adults, with practical implications for healthcare professionals.
Multimorbidity affects up to 65%-98% of individuals over the age of 65, with prevalence increasing with age and socioeconomic disadvantage. The most common combinations include cardiovascular disease, diabetes, osteoarthritis, and depression. Multimorbidity is associated with increased hospitalizations, polypharmacy, functional decline, and healthcare costs. Data from large cohort studies, such as the UK Biobank and the US National Health and Aging Trends Study, confirm substantial disease burden and underscore the necessity for innovative management strategies. The global demographic shift toward an aging population further amplifies the urgency of addressing multimorbidity in clinical practice.
The pathophysiology of multimorbidity is multifactorial, involving age-related physiological changes, chronic inflammation, genomic instability, and impaired homeostatic reserve. Shared mechanistic pathways, such as the metabolic syndrome, oxidative stress, and immune dysregulation, contribute to the clustering of chronic diseases. For example, insulin resistance and chronic low-grade inflammation may simultaneously predispose to diabetes, cardiovascular disease, and cognitive impairment. Understanding these interconnected mechanisms is essential for developing holistic and mechanism-based interventions, as opposed to isolated disease-specific treatments.
Risk factors for multimorbidity include advanced age, lower socioeconomic status, physical inactivity, poor nutrition, obesity, and environmental exposures. Social determinants of health, such as education, income, and access to care, play significant roles in disease clustering and outcomes. Genetic predisposition, cumulative life stress, and early-life adversity may also increase vulnerability. The INTERHEART and Whitehall II studies have illustrated the cumulative impact of modifiable and non-modifiable risk factors on the development of multimorbidity, emphasizing the need for comprehensive risk assessment in older adults.
Multimorbidity presents with heterogeneous clinical features, often characterized by overlapping symptoms, atypical presentations, and complex medication regimens. Older adults may experience cumulative functional impairment, frailty, cognitive decline, and reduced resilience to stressors. The interplay between conditions can exacerbate symptom burden (e.g., dyspnea from heart failure complicating diabetes-related neuropathy) and increase the risk of adverse drug events. Patterns of multimorbidity may influence clinical priorities, necessitating individualized care planning and regular reassessment of goals.
Diagnosing multimorbidity requires a comprehensive, patient-centered assessment, including detailed medical history, functional evaluation, cognitive screening, and review of current medications. Validated tools such as the Charlson Comorbidity Index and Cumulative Illness Rating Scale are commonly used to quantify disease burden. Geriatric assessment frameworks, incorporating physical, psychological, social, and environmental domains, are essential for identifying unmet needs and guiding care coordination. Diagnostic challenges include distinguishing disease-related symptoms from age-related changes and avoiding both over- and under-diagnosis.
Effective management of multimorbidity necessitates integrated care, involving multidisciplinary teams (MDTs) comprising physicians, nurses, pharmacists, social workers, and allied health professionals. Key principles include prioritization of patient goals, deprescribing to minimize polypharmacy, and individualized care plans that balance benefits and risks. Non-pharmacological interventions such as physical rehabilitation, nutrition optimization, and psychosocial support are integral. Shared decision-making and advance care planning enhance person-centered care, particularly in the context of limited life expectancy or frailty.
Recent advances in multimorbidity management include the development of digital health platforms, remote monitoring, and artificial intelligence-driven care coordination tools. Programs such as the Patient-Centered Medical Home (PCMH) and Geriatric Resources for Assessment and Care of Elders (GRACE) have demonstrated improved outcomes, including reduced hospitalizations and higher patient satisfaction. Pharmacogenomics and precision medicine approaches are being explored to tailor treatments and reduce adverse effects in complex patients. Ongoing clinical trials are evaluating integrated disease management models and novel therapeutics targeting shared pathophysiological pathways.
Recent guidelines from organizations such as the National Institute for Health and Care Excellence (NICE) and American Geriatrics Society emphasize holistic assessment, explicit goal setting, and regular medication review for older adults with multimorbidity. Individualized care plans, coordination across care settings, and attention to social determinants are prioritized. Deprescribing, fall prevention, and cognitive screening are recommended as routine components of care. Guidelines highlight the importance of communication between MDT members and engagement of patients and caregivers in care planning.
Managing multimorbidity in older adults is a complex and evolving challenge that demands a shift from disease-centric to patient-centered, integrated care models. Evidence supports that multidisciplinary, coordinated approaches improve functional outcomes, reduce hospitalizations, and enhance quality of life. Ongoing research into mechanistic pathways, digital health innovations, and precision medicine will further refine multimorbidity management. For clinicians, adopting guideline-based integrated care strategies and fostering collaborative, person-centered partnerships remain central to optimizing care for older adults with multimorbidity.
