Drawing on recent guidelines and contemporary PubMed-indexed literature, the article elucidates the pathophysiological underpinnings of postoperative functional decline and highlights practical, evidence-based strategies for accelerating recovery, with a focus on multidisciplinary approaches relevant to modern surgical practice.
Major surgical procedures, encompassing a wide spectrum from oncologic resections to complex cardiovascular operations, pose significant challenges to perioperative care teams due to risks of delayed functional recovery. Accelerating functional restoration is not only vital for minimizing postoperative complications and reducing healthcare costs but also for ensuring optimal patient-centered outcomes. Recent advances in perioperative medicine, anesthesiology, and rehabilitation have prompted a paradigm shift toward proactive, mechanism-driven recovery protocols. This article provides a comprehensive overview of the epidemiology, pathophysiology, risk stratification, clinical features, diagnostic approaches, treatment modalities, and guideline-aligned management strategies in the context of functional recovery post-major surgery.
Delayed functional recovery remains a common postoperative challenge, affecting up to 30% of patients undergoing major abdominal, orthopedic, or cardiothoracic surgery. The global burden is amplified by population aging and increasing surgical complexity, with older adults and those with comorbidities disproportionately impacted. Prolonged functional impairment is associated with increased risks of hospital-acquired complications, institutionalization, and mortality. Epidemiological studies underscore the economic impact, with extended hospital stays and readmissions contributing substantially to healthcare costs worldwide.

Enhanced Recovery After Surgery (ERAS) protocols have revolutionized perioperative care by integrating evidence-based practices across surgical pathways. Innovations such as prehabilitation structured exercise and nutritional optimization prior to surgery demonstrate improved postoperative functional outcomes and reduced complication rates. Novel pharmacologic agents targeting inflammation or mitochondrial dysfunction are under investigation. Wearable technology and remote monitoring enable real-time assessment of patient activity and facilitate personalized recovery trajectories. Furthermore, digital health platforms are increasingly employed to deliver tele-rehabilitation and support patient engagement beyond hospital discharge.
Contemporary clinical guidelines, including those from the American College of Surgeons and ERAS Society, advocate for standardized perioperative pathways emphasizing early mobilization, multidisciplinary care coordination, and proactive risk assessment.
