Unexplained weight loss (UWL), defined as a loss of more than 5% of the patient's body weight within six months, is a common yet perplexing clinical presentation. It can be the first sign of a serious underlying condition, necessitating a comprehensive and systematic approach to its evaluation.
The initial evaluation should focus on a detailed history and physical examination. It is crucial to determine whether the weight loss is intentional or unintentional, as the latter is more concerning. A thorough medication review, assessment of dietary habits, and psychosocial evaluation can often reveal the cause of weight loss.
If the initial evaluation does not provide a diagnosis, laboratory and imaging studies may be helpful. Basic tests include a complete blood count, metabolic panel, thyroid function tests, and urinalysis. Imaging studies, such as computed tomography (CT) scans, may be indicated based on the patient's symptoms and risk factors.
Specialist referral should be considered when the cause of weight loss remains elusive despite an exhaustive workup. Gastroenterologists, endocrinologists, oncologists, and psychiatrists are often involved in the evaluation of UWL, depending on the suspected underlying condition.
Regular follow-up is crucial in patients with UWL. Even if an initial workup is negative, patients should be monitored for new symptoms or changes in their condition. Some causes of weight loss, such as malignancies, may not become apparent until later in the disease course.
Unexplained weight loss is a challenging clinical scenario that requires a systematic and comprehensive approach. A thorough history and physical examination, appropriate laboratory and imaging studies, specialist referrals, and vigilant follow-up can help clinicians uncover the underlying cause and initiate appropriate management.
