This review synthesizes current literature on the epidemiology, pathophysiology, risk factors, clinical features, diagnostic approaches, management strategies, recent advances, and guideline recommendations relevant to nursing-driven recovery models. Clinical insights are provided on the mechanisms through which nursing interventions directly impact functional recovery, delineating practical implications for the healthcare team.
The complexity of medical and surgical conditions often results in prolonged functional impairments and increased dependency, posing significant challenges for healthcare professionals. Nursing-driven recovery pathways have gained traction as structured, protocolized approaches designed to facilitate early mobilization, promote patient autonomy, and foster multidisciplinary collaboration. These pathways are underpinned by the principle that nurses, due to their continuous patient contact and holistic care perspective, are uniquely positioned to identify barriers, coordinate interventions, and monitor progress towards functional independence. The adoption of such pathways aligns with contemporary healthcare priorities improving quality, reducing length of stay, and minimizing complications associated with immobility and dependency.
The global burden of disability and functional decline is considerable, particularly among the elderly, post-operative patients, and those with chronic illnesses. Hospital-acquired disability occurs in 30–60% of older adults during hospitalization, often resulting in loss of independence and increased institutionalization rates. Functional decline leads to higher morbidity, extended hospital stays, readmissions, and substantial healthcare costs. Diagnosis of functional decline and assessment of recovery potential require multidimensional evaluation. Standardized instruments, such as the Katz Index of Independence in ADLs, Lawton Instrumental Activities of Daily Living Scale, and FIM, provide quantitative measures of functional status. Nursing-driven pathways advocate for routine functional screening upon admission and at regular intervals. Nursing-driven recovery pathways typically feature protocols for early mobilization, progressive exercise, cognitive and psychosocial support, nutritional optimization, and patient education. Nurses play a central role in coordinating care, monitoring progress, and adjusting interventions in real time. Strategies such as bedside mobility rounds, ambulation protocols, and delirium prevention bundles have demonstrated efficacy in improving outcomes. Interprofessional collaboration with rehabilitation specialists, pharmacists, dietitians, and social workers is integral to the success of these pathways.
Recent advancements in recovery science include the integration of digital health technologies, wearable sensors, and tele-rehabilitation to facilitate real-time monitoring and remote support. Enhanced Recovery After Surgery (ERAS) protocols, which emphasize nursing-led interventions, have shown significant reductions in postoperative complications and expedited return to baseline function. Emerging evidence supports the use of motivational interviewing, mindfulness-based interventions, and virtual reality for cognitive and emotional rehabilitation. Continuous quality improvement initiatives and implementation science are increasingly leveraged to refine pathway components and drive sustainable practice change.
International and national guidelines increasingly endorse nursing-driven pathways as standard practice in recovery-oriented care. The American Nurses Association, European Society for Clinical Nutrition and Metabolism, and other bodies advocate for routine functional assessment, early mobilization, and patient-centered goal setting. Guidelines emphasize the importance of nurse-led care coordination, interdisciplinary collaboration, and documentation of functional outcomes. Nursing-driven recovery pathways represent a paradigm shift in the rehabilitation and restoration of functional independence. The integration of recent advances and adherence to guideline recommendations further enhance their impact. Ongoing research and quality improvement efforts are needed to optimize pathway components, address barriers to implementation, and expand their applicability across diverse clinical populations.
