Frailty is a prevalent, multidimensional syndrome in older adults, characterized by decreased physiological reserves and increased vulnerability to adverse health outcomes. Early identification of frailty in primary care is critical to optimize prevention, management, and resource allocation. This review synthesizes current evidence on frailty screening, explores the underlying mechanisms, and discusses the clinical utility, recent advances, and guideline recommendations for implementing frailty assessment in primary care settings. The article emphasizes practical approaches, validated screening tools, risk stratification strategies, and outlines the challenges and future directions in integrating frailty screening into routine clinical practice.
Frailty constitutes a major public health concern as populations age globally. Defined as a state of increased vulnerability resulting from age-associated declines in physiological function, frailty predisposes individuals to falls, disability, hospitalization, and mortality. In primary care, timely frailty screening enables clinicians to identify at-risk patients, tailor interventions, and coordinate multidisciplinary care. Given the complex and heterogeneous nature of frailty, there is a growing emphasis on standardized, evidence-based screening approaches to accurately stratify risk and guide management. This review aims to provide clinicians with a comprehensive overview of frailty screening, incorporating recent literature, mechanistic insights, and practical implications for primary care.
Frailty affects a significant proportion of adults aged 65 years and older, with prevalence estimates ranging from 10% to 25% depending on the population and screening criteria used. The burden of frailty is projected to increase as life expectancy rises worldwide. Epidemiological studies indicate that frailty is associated with a two- to three-fold increase in the risk of falls, functional decline, institutionalization, and all-cause mortality. Notably, the transition from robust to pre-frail and frail states often occurs over several years, offering an opportunity for early intervention. Variations in prevalence are observed across ethnicities, socioeconomic strata, and healthcare settings, highlighting the need for context-specific screening strategies in primary care.
The pathophysiology of frailty is multifactorial and involves complex interactions among musculoskeletal, neuroendocrine, immune, and metabolic systems. Hallmarks include sarcopenia (loss of muscle mass and strength), chronic low-grade inflammation (\"inflammaging\"), hormonal dysregulation (e.g., reduced anabolic hormones), oxidative stress, and impaired energy metabolism. These biological changes contribute to diminished homeostatic reserve and resilience to stressors such as infection, surgery, or medication changes. Importantly, frailty is distinct, yet often overlaps with comorbidity and disability, necessitating careful clinical assessment to delineate these syndromes and guide individualized care.
Major risk factors for frailty include advanced age, female sex, low socioeconomic status, polypharmacy, chronic diseases (such as cardiovascular disease, diabetes, chronic kidney disease), malnutrition, cognitive impairment, and physical inactivity. Social isolation, depression, and sensory deficits (vision and hearing loss) also contribute to the development and progression of frailty. Recognizing these risk factors enables clinicians to proactively identify individuals who may benefit from targeted screening and preventive interventions in primary care.
Frailty manifests clinically as a syndrome of decreased strength, endurance, and physiological function. Cardinal features include unintentional weight loss, weakness, exhaustion, slow walking speed, and low physical activity collectively operationalized in the widely used Fried Frailty Phenotype. Additional signs may include balance impairment, recurrent falls, cognitive decline, and increased susceptibility to delirium or adverse drug reactions. Early clinical recognition is essential, as frail individuals often present with non-specific symptoms and are at elevated risk for rapid health deterioration following minor stressors.
Diagnosis of frailty in primary care relies on validated screening tools and clinical assessment. The Fried Frailty Phenotype and the Rockwood Clinical Frailty Scale are among the most widely adopted instruments. The phenotype model requires assessment of five criteria: unintentional weight loss, weakness (grip strength), self-reported exhaustion, slow gait speed, and low activity. Alternatively, the deficit accumulation approach, as used in the Rockwood scale, considers a broader array of deficits across physical, psychological, and social domains. Other pragmatic tools for primary care include the PRISMA-7 questionnaire, FRAIL scale, and electronic frailty indices derived from routine health records. Selection of an appropriate tool depends on practice context, available resources, and clinician familiarity.
Management of frailty is multifaceted, with a focus on comprehensive geriatric assessment, individualized care planning, and multidisciplinary collaboration. Key interventions include tailored physical exercise programs (particularly resistance and balance training), nutritional optimization (addressing protein and caloric deficits), medication review to minimize polypharmacy, management of comorbidities, cognitive and psychosocial support, fall prevention strategies, and advance care planning. Evidence supports the benefit of multifactorial interventions, particularly when initiated early in the course of frailty. Regular follow-up and dynamic reassessment are crucial to monitor progress and adjust care as needed.
Recent advances in frailty research have focused on precision screening tools leveraging electronic health records, machine learning algorithms for risk prediction, and digital health interventions to support self-management. Novel biomarkers, such as inflammatory cytokines and muscle function tests, are under investigation to enhance diagnostic specificity. Emerging therapies target underlying mechanisms, including anabolic agents, anti-inflammatory drugs, and interventions to modulate the gut microbiome. Pilot studies are exploring the feasibility of telemedicine-based frailty assessments and remote monitoring to expand access in underserved populations. Integration of patient-reported outcomes and shared decision-making frameworks is also gaining traction in the comprehensive management of frailty.
International guidelines, including those from the British Geriatrics Society, NICE, and the American Geriatrics Society, endorse routine frailty screening for older adults in primary care. Recommendations emphasize the use of validated, feasible tools, incorporation of frailty status into individualized care plans, and coordination with geriatric specialists as needed. Screening is particularly advised prior to major medical interventions, hospital discharge, or when there is evidence of functional decline. Guidelines highlight the importance of ongoing clinician education, adequate resource allocation, and system-level support to sustain frailty screening initiatives.
Frailty screening in primary care represents a cornerstone of proactive geriatric care, enabling early identification and intervention for at-risk older adults. Utilization of validated tools, multidisciplinary management, and adherence to guideline-based practices are essential to improve health outcomes, reduce healthcare utilization, and enhance quality of life for frail individuals. Ongoing research into novel diagnostics, digital technologies, and mechanism-based therapies holds promise for refining frailty assessment and management in the future. Primary care clinicians play a pivotal role in leading these efforts, ensuring that frailty screening becomes an integral component of comprehensive, patient-centered care for the aging population.
