Preserving kidney function in high-risk populations is a cornerstone of preventive nephrology, with significant implications for morbidity and mortality among vulnerable patient groups. This article synthesizes recent evidence and guideline-based recommendations for clinicians, focusing on epidemiology, pathophysiology, risk factors, clinical presentation, diagnostic strategies, and management protocols tailored to those at elevated risk for chronic kidney disease (CKD). Emerging therapeutic advances and expert insights are highlighted to inform best practices and optimize patient outcomes.
Chronic kidney disease constitutes a global public health concern, with prevalence rising in parallel with aging populations and increasing rates of diabetes, hypertension, and cardiovascular disease. High-risk groups such as those with diabetes mellitus, hypertension, cardiovascular disease, autoimmune disorders, and a history of nephrotoxic exposures require targeted strategies for kidney function preservation. This review aims to provide clinicians with an updated, evidence-based overview of guideline-endorsed approaches for risk mitigation and early intervention, emphasizing practical applications in everyday clinical practice.
CKD affects approximately 10–15% of adults worldwide, with higher prevalence observed among individuals with diabetes, hypertension, and advancing age. The burden is particularly pronounced in socioeconomically disadvantaged populations and certain ethnic groups, including African Americans, Hispanics, and Indigenous peoples. Disease progression often remains asymptomatic until advanced stages, underscoring the necessity for early identification and intervention in high-risk cohorts. The resultant morbidity includes increased cardiovascular events, hospitalizations, and premature mortality, making CKD a significant contributor to health system costs and patient quality of life impairment.
CKD is characterized by sustained structural or functional renal impairment lasting more than three months. Key mechanisms include glomerular hyperfiltration, tubulointerstitial fibrosis, endothelial dysfunction, and maladaptive activation of the renin-angiotensin-aldosterone system (RAAS). In high-risk populations, hyperglycemia, hypertension, chronic inflammation, and oxidative stress precipitate podocyte injury, proteinuria, and progressive nephron loss. Repeated nephrotoxic insults such as nonsteroidal anti-inflammatory drug (NSAID) exposure or contrast-induced nephropathy exacerbate underlying vulnerability, accelerating decline toward end-stage kidney disease (ESKD).
Major risk factors for CKD progression include poorly controlled diabetes and hypertension, proteinuria, obesity, dyslipidemia, smoking, family history of CKD, older age, and exposure to nephrotoxins. Specific high-risk populations encompass patients with systemic diseases (e.g., systemic lupus erythematosus, HIV), recipients of solid organ transplants, and those with a history of acute kidney injury (AKI). Socioeconomic status, limited healthcare access, and genetic predispositions further modulate individual risk profiles.
Early CKD is frequently asymptomatic, with subtle manifestations such as nocturia, fatigue, or mild edema. As disease advances, patients may develop hypertension, anemia, electrolyte abnormalities, bone-mineral disorders, and overt signs of uremia. In high-risk groups, vigilant surveillance for proteinuria, declining estimated glomerular filtration rate (eGFR), and blood pressure trends is paramount for timely intervention.
Screening high-risk populations involves annual assessment of serum creatinine (for eGFR estimation), urine albumin-to-creatinine ratio (ACR), and blood pressure monitoring. Ultrasonography may be warranted for structural evaluation in select cases. Diagnostic criteria for CKD include eGFR <60 mL/min/1.73 m², or markers of kidney damage (e.g., albuminuria, abnormal imaging, or histology) persisting for >3 months. Identifying rapidly progressive disease, secondary causes, or concurrent complications prompts specialist referral for further evaluation.
Management focuses on modifiable risk factor control and slowing disease progression. Glycemic optimization (target HbA1c <7% for most), stringent blood pressure control (target <130/80 mmHg), and RAAS blockade (ACE inhibitors or ARBs) are foundational. Statin therapy is recommended for cardiovascular risk reduction. Lifestyle interventions weight management, dietary sodium and protein restriction, smoking cessation, and regular physical activity are integral. Avoidance of nephrotoxic medications and prompt recognition of AKI episodes are critical in high-risk individuals. Multidisciplinary care involving nephrologists, dietitians, and primary care teams enhances outcomes.
Sodium-glucose cotransporter-2 (SGLT2) inhibitors have demonstrated robust renoprotective effects in both diabetic and non-diabetic CKD patients, reducing progression to ESKD and cardiovascular events. Non-steroidal mineralocorticoid receptor antagonists (e.g., finerenone) offer further benefit in select populations. Advances in biomarker discovery, genetic risk stratification, and personalized medicine are poised to refine risk prediction and therapeutic tailoring. Remote monitoring and digital health interventions show promise for improving adherence and early detection of deterioration in at-risk groups.
International guidelines (KDIGO, ADA, AHA) endorse annual screening for kidney dysfunction in high-risk populations, aggressive management of diabetes and hypertension, and early initiation of RAAS blockade in those with albuminuria. SGLT2 inhibitors are recommended for patients with type 2 diabetes and CKD independent of glycemic control, and consideration of non-steroidal mineralocorticoid antagonists in appropriate cases. Individualized targets, shared decision-making, and patient education are emphasized to optimize adherence and outcomes. Timely referral to nephrology is advised for advanced CKD, refractory hypertension, or rapid progression.
Preservation of kidney function in high-risk populations requires a proactive, evidence-based approach integrating early identification, risk factor modification, and implementation of emerging therapies. Adherence to contemporary guideline recommendations, supported by multidisciplinary collaboration and patient engagement, is essential to mitigating CKD burden and improving long-term outcomes. Ongoing research and innovation will continue to shape the clinical landscape, offering new opportunities to enhance kidney health in vulnerable patient cohorts.
