Abstract

Functional neurological symptoms, formerly referred to as conversion disorders, present significant diagnostic and therapeutic challenges in psychiatric and neurological practice. This review article synthesizes recent evidence, discusses epidemiology, pathophysiology, risk factors, clinical features, diagnostic strategies, and current management approaches for functional neurological symptoms (FNS), with emphasis on case-based insights and guideline recommendations. Clinically relevant advances and emerging therapies are highlighted to enhance practical understanding for healthcare professionals.

Introduction

Functional neurological symptoms (FNS) encompass a spectrum of neurological complaints—such as weakness, movement disorders, sensory symptoms, and non-epileptic seizures—that are inconsistent with recognized neurological disease but are associated with significant distress and disability. Once termed "hysteria" or "conversion disorder," FNS is now understood as a genuine neuropsychiatric condition. This article explores FNS through the lens of recent literature and clinical case discussions, aiming to equip doctors and healthcare professionals with evidence-based tools for improved diagnosis and management.

Epidemiology / Disease Burden

The global prevalence of FNS is estimated to be between 2% and 5% in neurology outpatient settings, making it one of the most common causes of neurological disability in young adults, particularly women. Hospital-based studies report that up to 30% of new neurology referrals may involve functional symptoms. The condition is associated with substantial healthcare utilization, frequent investigations, and significant impacts on quality of life and occupational functioning. Comorbid psychiatric conditions such as depression and anxiety are frequently observed, further complicating disease burden.

Pathophysiology

The pathophysiology of FNS is multifactorial, involving aberrant neural network activity, altered sensorimotor integration, and maladaptive cognitive processes. Neuroimaging studies have identified abnormal connectivity between limbic regions and motor cortices, supporting a model of disrupted top-down control. Functional MRI has demonstrated excessive activation in regions associated with emotion and self-monitoring during symptom provocation. These findings suggest that FNS arises from a complex interaction of psychological stressors, attentional mechanisms, and neurobiological vulnerabilities, rather than conscious feigning or malingering.

Risk Factors

Risk factors for FNS include a history of psychological trauma, adverse childhood experiences, and high levels of emotional arousal. Personality traits such as alexithymia and suggestibility may contribute to vulnerability. Coexisting psychiatric disorders—most commonly mood and anxiety disorders—are prevalent. Medical comorbidities, chronic pain syndromes, and prior exposure to illness models (family or cultural) can also increase susceptibility. Recent studies highlight the role of psychosocial stressors and acute life events in precipitating symptom onset.

Clinical Features

FNS manifests with a broad range of symptoms, including functional motor weakness, tremor, dystonia, gait disturbances, and non-epileptic attack disorder (NEAD). Sensory symptoms—such as numbness, blindness, or deafness—are also reported. Key clinical features include inconsistency of symptoms on examination, incongruity with recognized neurological patterns, and positive clinical signs (e.g., Hoover's sign, entrainment test). Symptoms often fluctuate and are influenced by attention, distraction, and suggestion. Case vignettes illustrate presentations such as sudden paralysis following psychological stress, or episodic seizures unaccompanied by EEG abnormalities.

Diagnosis

Diagnosis of FNS is clinical, based on positive signs and the exclusion of organic neurological disease. The DSM-5 outlines criteria that emphasize demonstration of incompatibility with recognized neurological conditions. Neuroimaging and electrophysiological studies are used to rule out structural lesions or epileptic activity, respectively. The use of functional tests (e.g., Hoover’s sign for leg weakness, tremor entrainment) confirms functional etiology. Early and clear communication of the diagnosis to patients, using a transparent and non-judgmental approach, is critical for engagement and prognosis.

Treatment & Management

Multidisciplinary management is the cornerstone of FNS treatment, involving neurologists, psychiatrists, psychologists, physiotherapists, and occupational therapists. Psychoeducation and validation of symptoms are essential first steps. Cognitive-behavioral therapy (CBT) tailored for FNS has the strongest evidence base, especially for non-epileptic attacks and motor symptoms. Physiotherapy focusing on retraining normal movement patterns is beneficial for motor presentations. Pharmacotherapy may be indicated for comorbid depression or anxiety but is not effective for core functional symptoms. Regular follow-up and goal-oriented rehabilitation improve long-term outcomes.

Recent Advances / Emerging Therapies

Recent advances in FNS research include the development of specialized functional movement disorder clinics and the integration of neuroimaging biomarkers for diagnostic clarification. Digital therapeutics and internet-based CBT platforms are expanding access to evidence-based interventions. Neurofeedback, transcranial magnetic stimulation (TMS), and mindfulness-based therapies are under investigation and show preliminary promise. Emerging data support early intervention and the use of multidisciplinary team-based approaches for optimizing recovery. Case studies highlight successful application of these modalities in clinical practice.

Guideline Recommendations

International guidelines from the American Psychiatric Association and the UK National Institute for Health and Care Excellence (NICE) recommend a stepwise approach: 1) early recognition and positive diagnosis using clinical signs, 2) clear communication of the diagnosis to the patient, 3) multidisciplinary rehabilitation with focus on functional restoration, and 4) management of comorbid psychiatric conditions. Avoidance of repeated unnecessary investigations is emphasized. Patient-centered care, shared decision-making, and regular review are highlighted as best practices for improving engagement and prognosis.

Conclusion

Functional neurological symptoms are a common and complex neuropsychiatric condition with significant implications for patient quality of life and healthcare systems. Advances in neuroimaging, evidence-based psychological therapies, and guideline-driven multidisciplinary management have improved diagnosis and outcomes. Ongoing research into pathophysiological mechanisms and novel interventions will further enhance care. Early recognition, destigmatization, and collaborative management remain central to optimizing prognosis for patients with FNS.
