This article critically reviews epidemiological trends, pathophysiological mechanisms, risk factors, clinical features, and diagnosis of visceral adiposity in later life, integrating the latest research on treatment, emerging therapies, and guideline recommendations. Special emphasis is placed on the clinical relevance of visceral adiposity patterns, their impact on age-related comorbidities, and the importance of proactive management in geriatric populations.
\nThe distribution of adipose tissue undergoes significant changes with advancing age, with a pronounced shift toward visceral fat accumulation. Unlike subcutaneous fat, visceral adiposity is metabolically active and is implicated in the pathogenesis of a range of age-associated disorders, including type 2 diabetes, cardiovascular disease, and certain malignancies. The identification and management of visceral adiposity in the elderly require an evidence-based, multidisciplinary approach due to its complex interplay with comorbidities and age-related physiological changes.

\nManagement of visceral adiposity in the elderly demands a comprehensive approach tailored to individual health status and comorbidities. Lifestyle modification remains the cornerstone, with structured physical activity and Mediterranean or DASH-style dietary patterns showing significant efficacy in VAT reduction. Resistance and aerobic exercise regimens improve muscle mass and attenuate visceral fat. Pharmacotherapy may be considered in select individuals, particularly those with metabolic syndrome or type 2 diabetes. Agents such as GLP-1 receptor agonists and SGLT2 inhibitors have demonstrated VAT-lowering effects and favorable metabolic profiles. In rare cases, bariatric surgery may be indicated, but careful risk-benefit assessment is crucial in geriatric populations.
\nEmerging therapies targeting visceral adiposity focus on modulating adipose tissue inflammation, mitochondrial function, and hormonal axes. Novel agents such as selective androgen receptor modulators, myostatin inhibitors, and anti-inflammatory biologics are under clinical investigation. Advances in molecular imaging and artificial intelligence-driven risk prediction models offer promise for early detection and personalized management. Lifestyle interventions delivered via telemedicine and digital health platforms also show potential for broadening access and improving adherence in older adults.
\nLeading societies, including the American Diabetes Association, Endocrine Society, and European Society of Cardiology, recommend routine screening for central obesity and metabolic syndrome in older adults with cardiovascular risk factors. Guidelines emphasize the use of waist circumference and imaging when feasible and advocate for individualized lifestyle interventions as first-line therapy. Pharmacologic agents should be selected based on comorbidity profile, tolerability, and potential benefits in VAT reduction. Multidisciplinary care, including nutrition, physical therapy, and psychological support, is endorsed for optimizing outcomes in this population.
\nVisceral adiposity in later life represents a clinically significant, yet often underrecognized, contributor to morbidity and mortality in older adults. Advances in understanding its pathophysiology, assessment, and management have enabled more effective, personalized approaches. Ongoing research into novel therapies and prevention strategies holds promise for reducing the burden of visceral adiposity and its complications in aging populations. Proactive, guideline-based management remains essential for optimizing health, function, and quality of life in geriatric patients facing the challenges of central obesity.
