Oral function plays a pivotal role in maintaining nutritional well-being among adult populations. Emerging evidence underscores the intricate relationship between oral health, masticatory efficiency, and overall nutritional status, with significant clinical consequences when oral function is compromised. This review synthesizes current literature, highlights epidemiological trends, elucidates underlying mechanisms, and discusses clinical implications for healthcare professionals. Practical recommendations, guideline-based approaches, and recent advances are also explored to optimize patient outcomes through interdisciplinary care.
The interdependence between oral function and nutritional well-being is increasingly recognized as a cornerstone of adult health. Oral dysfunction, encompassing impaired mastication, swallowing difficulties, and dental disorders, can precipitate malnutrition, exacerbate systemic diseases, and diminish quality of life. Given the rising prevalence of chronic diseases and an aging global population, understanding these connections is critical for clinicians managing adult patients in both primary and specialty care settings.
Globally, oral health disorders remain among the most prevalent chronic conditions, with edentulism, periodontitis, and dental caries affecting millions of adults. According to the Global Burden of Disease Study, untreated dental caries and severe periodontitis rank among the top causes of years lived with disability. In older adults, the prevalence of partial or complete tooth loss exceeds 30% in many regions, correlating strongly with reduced dietary diversity and increased risk of protein-energy malnutrition. Studies in high-income and low- to middle-income countries alike report that poor oral function is associated with frailty, sarcopenia, and increased morbidity.
The mechanisms linking oral dysfunction to deteriorating nutritional status are multifactorial. Tooth loss impairs masticatory performance, limiting the ability to chew fibrous, nutrient-rich foods such as fruits, vegetables, and meats. Xerostomia (dry mouth), commonly seen in polypharmacy and in certain systemic diseases, further impairs bolus formation and swallowing. Additionally, oral pain and mucosal lesions may drive individuals toward softer, calorie-dense, but micronutrient-poor diets. Chronic inflammation associated with periodontitis also contributes to systemic inflammatory burden, potentially exacerbating metabolic dysregulation and affecting nutrient metabolism. These pathophysiological changes often result in a downward spiral of poor intake, weight loss, and increased disease vulnerability.
Numerous risk factors for compromised oral function and subsequent nutritional deficits have been identified. Age-related physiological changes, such as reduced salivary flow and decreased muscle mass, predispose older adults to oral dysfunction. Socioeconomic status, limited access to dental care, comorbidities (e.g., diabetes, stroke, neurodegenerative diseases), tobacco and alcohol use, and poor oral hygiene are significant contributors. Polypharmacy, particularly the use of anticholinergic agents, further increases xerostomia risk. Nutritional risk is accentuated in populations with cognitive impairment or physical disabilities, where self-care is compromised.
Clinical manifestations of impaired oral function range from observable tooth loss, ill-fitting dentures, oral mucosal lesions, and reduced bite force to subjective complaints such as pain, discomfort, and avoidance of certain foods. Patients may report difficulties with chewing, swallowing (dysphagia), or altered taste perception. Nutritional consequences manifest as unintended weight loss, reduced body mass index, micronutrient deficiencies (notably iron, vitamin B12, and folate), and functional decline. In advanced cases, malnutrition can potentiate frailty, delayed wound healing, and increased susceptibility to infections.
Comprehensive assessment of oral function in adults requires a multidisciplinary approach. Clinical oral examination, assessment of dental status (including number and condition of teeth, presence of prostheses), masticatory performance tests (e.g., color-changeable chewing gum, sieving tests), and evaluation of salivary flow are fundamental. Nutritional status should be concurrently assessed using validated tools such as the Mini Nutritional Assessment (MNA) or Subjective Global Assessment (SGA). Laboratory investigations may be warranted to detect subclinical deficiencies. Collaboration with dietitians, speech-language pathologists, and dental specialists is recommended for holistic evaluation.
Management strategies focus on restoring oral function and optimizing nutritional intake. Dental interventions—including restorative procedures, prosthodontics (dentures, implants), and periodontal therapy—are foundational. Addressing xerostomia involves medication review, salivary substitutes, and stimulation techniques. Nutritional counseling should emphasize texture-modified diets, supplementation, and strategies to enhance palatability and intake. Intensive oral hygiene education and regular follow-up are essential to prevent further deterioration. For patients with dysphagia, referral for swallowing rehabilitation may be indicated.
Recent advances include the development of digital dentistry and CAD/CAM technologies, which enable rapid and precise fabrication of dental prostheses, improving masticatory efficiency and patient satisfaction. Bioactive materials and regenerative approaches show promise in periodontal therapy and tooth replacement. Novel salivary substitutes and pharmacologic agents targeting glandular function are under investigation. Additionally, integrated care models leveraging tele-dentistry and multidisciplinary teams have demonstrated improved outcomes in at-risk populations.
Current guidelines from the World Health Organization and professional dental associations emphasize the importance of routine oral health assessments in adult and geriatric care. Early identification of oral dysfunction and nutritional risk should prompt timely intervention. Interdisciplinary collaboration among physicians, dentists, dietitians, and allied health professionals is advocated to address the complex interplay between oral health and nutrition. Patient education and community-based prevention strategies are highlighted as essential components of comprehensive care.
The evidence unequivocally demonstrates that preserving oral function is integral to maintaining adequate nutrition and overall health in adult populations. Clinicians should adopt a proactive, multidisciplinary approach to identify and manage oral dysfunction, with particular attention to vulnerable groups. Emerging technologies and collaborative care models offer new avenues to enhance patient outcomes. Ongoing research and education are essential to bridge gaps in practice and ensure that oral health remains central to holistic adult care.
