Furthermore, patient and family engagement strategies, such as shared decision-making and self-management support, can foster a culture of PCC. Finally, ongoing evaluation and feedback mechanisms can ensure the continuous improvement of PCC practices.
In conclusion, PCC models hold promise for improving patient satisfaction and outcomes. However, their implementation requires overcoming significant challenges and necessitates a comprehensive and sustained approach. As healthcare systems continue to evolve, further research is needed to refine these models and optimize their integration into practice.
